If you take three or more medicines, there is an appointment worth booking that almost nobody books. Not a repeat script. A review.

Medicines accumulate quietly. One started years ago for a reason that has since passed. One added by a specialist who did not have the full list in front of them. Nothing wrong with any individual decision, and no one ever looking at the whole thing together.

The gaps are usually supplements and anything off a pharmacy shelf, because people do not think of those as medicine and leave them out. They interact all the same.

So bring everything. The actual boxes, or photographs of them, including the ones you stopped taking and never mentioned to anybody.

Sometimes the outcome is fewer medicines. Sometimes it is just knowing exactly why you take each one, which is worth the appointment on its own.

The number on the thermometer is the thing everybody watches and the thing that tells you least.

A high reading in someone who is alert, drinking and recognisably themselves is less worrying than a modest one in someone drowsy and not quite right. Height of fever is a poor guide to how unwell a person actually is.

Medicine for a fever is for comfort. It helps someone rest and keep drinking, which is what actually matters. It is not a race to get a number down.

What to watch instead is behaviour. Drowsiness, confusion, not drinking, not passing urine, or a child who cannot be roused and settled.

And the ones that do not wait: any fever at all in a baby under three months, a stiff neck, a rash that does not fade when you press a glass against it, difficulty breathing, or a fever that has run past three days.

If you are going anywhere in December, this is the week it stops being early and starts being sensible.

Four to six weeks before departure is the window travel medicine is built around. Some vaccine courses run over several weeks rather than being a single shot, and a yellow fever certificate only becomes valid ten days after the vaccination. Leave that one late and you can be refused entry at a border.

A travel consultation works off your actual trip. Where you are going, which route, what dates, and your own health history. You leave with what you need rather than the full menu, plus malaria prophylaxis if the destination calls for it.

Mozambique, Zanzibar, Thailand and the Kruger all need something different.

Message us with your destination and your departure date and we will tell you what applies.

A lot of what sends people to a casualty department on a weekend is ordinary general practice work, and they only find that out after three hours and a bill.

A thirty-second phone call tells you which one this is.

We are not going to pretend everything belongs here. Chest pain, difficulty breathing, serious bleeding, a head injury, stroke symptoms, or anyone who has collapsed: that is an emergency department, immediately, and do not drive yourself. If you phone us about one of those, that is exactly what we will tell you.

Everything in between is worth asking about, especially on a Saturday or a Sunday when the default assumption is that nothing else is open.

We are open seven days a week, including public holidays.

Heat exhaustion and heatstroke get used interchangeably. They are not the same thing, and the difference decides what you do next.

Heat exhaustion looks like heavy sweating, cool clammy skin, weakness, nausea, headache and dizziness. Shade, lying down, cooling off and fluids. It usually settles.

Heatstroke looks like confusion, agitation, slurred speech or collapse, with a very high temperature. That is an emergency and it is not something to manage in a lounge with a wet cloth.

The myth worth dropping is the one most people were taught: that someone with heatstroke stops sweating. Not reliably. Somebody who collapses while exercising in the heat can still be drenched and already be in serious trouble, which is exactly how this gets missed.

Confusion is the dividing line, not the sweating. If someone is not making sense, stop trying to cool them at home and get help immediately.

"Come back if it gets worse" is the most misread sentence in medicine. It sounds like a polite way of saying there is nothing wrong. It is not.

Most diagnoses in general practice are probabilities rather than certainties. A doctor seeing you on day two of an illness is often looking at something that has not declared itself yet, and time is genuinely the most useful test available. Sending you home with instructions is a plan, not a dismissal.

A good version of that plan is specific. What to watch for, by when, and where to go. If you left without all three, you left with half an instruction, and you are entitled to ask for the rest.

The best question you can ask in any consultation is some version of "what would change your mind?" A useful answer names the thing that would shift the diagnosis: a fever returning after settling, symptoms worsening after improving, a new symptom appearing. Then you know exactly what you are watching for instead of guessing.

And if it does get worse, come back. Returning is not wasting anyone's time. It was the second half of the assessment all along.

Most back pain settles on its own. What you do in the first two days mostly decides how fast.

Bed rest is out of date. It was standard advice for a long time and it makes most back pain worse rather than better. Short, frequent movement beats lying still, even when movement is the last thing you feel like.

Returning to normal activity sooner, in a reduced way, shortens recovery. Waiting until the pain has gone before doing anything tends to lengthen it.

On pain relief, check what you are already taking before you add anything. Paracetamol hides inside a lot of combination products and stacking it is easier than people expect.

Then the part that is not reassuring, and should not be. See someone straight away if there is any change in bladder or bowel control, numbness around the saddle area, or new weakness in a leg or foot. Also if there is a fever alongside the back pain, or if it followed a serious fall or a crash.

Those are uncommon. They are also the ones where waiting costs the most.

If you are joining a yacht, a cruise ship or a merchant vessel, you need a seafarer medical from a SAMSA-accredited practitioner. Not any doctor, and not your regular GP.

We have one here in Hatfield.

The examination covers a medical history review, a physical examination, vision and hearing testing, urinalysis, and a chest X-ray where the regulations require it. Radiology is in this building, so that does not become a second appointment somewhere else.

Worth knowing: seafarer medical certificates now run for two years from date of issue rather than one, unless an exclusionary condition applies.

Message us with where you are joining and when you sail, and we will tell you what is needed before you book anything.
