Always lekker to have a 6 canal molar with actually 6 canals and not just a prep-able isthmus! Did you know South Africa has quite a large proportion of separate middle distals!

Follow up CBCTs... I don't take CBCTs for follow-up as the radiation exposure doesn't justify it purely just for follow-up, but every now and then when a patient comes for a second time in the same quadrant then I get a CBCT follow-up yippie! 14 month follow-up on the 34 and 33.

File removal on a 12, the patient had a separated instrument present with a previous removal attempted.
Image 1 is pre-op with some calcium hydroxide present,
Image 2 is the pre-op superimposed on after the removal to show how little structure was further removed, just the little triangle apically.

The separated instrument was removed using ultrasonics, preparation completed with WOG, and obturation with Ceraseal and single cone/CLC.
The present defect was filled with composite to increase the resistance of the tooth, I was hesitant to place GP all the way coronally as GP lends no strength to the tooth.

A lekker apical split of the MB2, these are difficult to negotiate on a 6... and even more so on a 7! 😅
Preparation with Hyflex 60.02 for the palatal, with good tugback on a #55 GP so I didn't pack any MTA.
Prep MB and DB with Excalibur 35.05 and MB2 split with edgefile x7 25.04 then by hand to #30.
Tip for these apical splits, do not finish preparing the main canal and then try and locate the split, it makes it very difficult if the main canal is very large to try and maneuver into the small split. Best is to stay around the same size in both canals and increase the prep sizes at the same time until you can access both easily and only then switch to engine instruments. In this case I did both the MB canals until #20 hand files before switching to my glider
Irrigation 6% Hypo and 17% EDTA.
All canals obturation with Ceraseal and single cone/CLC.
Isolation with trusty rubberdam as always.

Quick case!
37 with a failed previous endodontic treatment, the patient reported that the tooth just never felt "right" after the first endo.
This case was done over two visits with de-obturation and medication for drainage the first appointment. After the first appointment the patient reported that this is the first time the tooth has ever not bothered him.
At the second appointment the preparation was completed with hand files to a size 60. Usually I would use the Hyflex 60.02 for this part but the canal was ledged right at the apical dog leg part of the canal. It was just light-years easier to do the last few files with hand than to try and pre-curve and insert the rotary file. I did engine preparation with Excalibur 45.05 final file and then proceeded with k-files to #60.
Irrigation with 6% hypo, 17% EDTA, Saline and 2% CHX. Activation with ultrasonics.
Obturation with ProRoot MTA apical plug and Ceraseal and WVC for the coronal two-thirds.
Restoration with Palfique bulk fill.

Lecture last month in Bloem, what a lovely bunch 😁 I really enjoyed it! If you haven't heard yet.... Zarc Excalibur is my new go to file. Like any file system, there isn't one file to fit every canal, but this system I use for about 80% of my cases. It is a more minimally invasive reciprocating file with great cutting efficiency. A two canal premolar, a separated file, and two retreats.

What, may you ask, was the most difficult part of my day? The ledged MB canal on the 26.

Random case selection... I just typed in a surname and ta-da you guys can have a look at some of my pretty lines. Nothing glamourous today, just some routine things. Maybe next time the random surname lottery will be more interesting.
And yes, the apexes on the 46 were really that large.
