Difficulty to open mouth

This is a real nightmare that can come from various sources.

1. You just are like that, and have always been like that. No traumatic incident, no operation. It is the way you are. This is very complex and difficult to treat. A x-ray of the joints of your jaw can help to identify if there is a mechanical (cartlidge) obstruction. There are some exercises that can be performed, but the cause of this needs to be identified.

2. Infection around impacted wisdom teeth. The restriction of opening will be a gradual onset. An x-ray can confirm this and sadly, removal of wizzies can’t solve the problem. Luckily, this TM joint problem can be treated.

3. Dislocation of one of the TM joint disks. This usually presents with clicking on opening or on closing the jaw. (with translation or reduction) This one is a real little bu**er. Treatment is complex and extreme cases can be operated on by a specific surgeon.

4. The secret is, to treat your TM joints with respect.

Do not get hit on the joint with a fist.
No not over stretch your mouth without reason.
Do not try to fit a tennis ball in your mouth. Respect it!

Sometimes I finks

Sometimes I sits and I finks (spelling mistakes intended)

These words we, as students, said in respect of the statue of a man, that appears to sit and think. ( The Thinker” or “Le Penseur). Sometimes I have these thoughts of life. The meaning of this all, and NO, I do not have a religious empty space that needs to be filled. I just think.

Are all the important facts about how to manage your health, that we are being fed by research and media, really that important. Do we really need to take all the supplements, keep track of out steps each day, work ourselves to death, watch our electronic wrist attached watches all day, just to be healthy, as heath is defined by media and internet research.

I am not insinuating that you must continue smoking and be a couch slouch and not take care of your mind and body. I am asking (thinking), can we not just live life with normal food and hard old school exercise, and still be healthy. Is it important to have that one little slightly skew tooth corrected with braces, and you must have toxins and strange fillers injected for little wrinkles and have to consume absurd amounts of tinned supplements, just to be healthy. (By the way, little wrinkles show wisdom, status and is sexy.). Will your life terminate into total chaos?

What if we just accept a bit of what life give us. Take care of what we were dealt, but do it well. Laugh a bit more and let that steroid pumped guy in his 4x4 cut in. Let him go, because he will NEVER reach a point where he will realize that that single one second he stole from you, has no benefit for him. What if, preserving your spiritual wellbeing, our fun in life, eat just enough (hara Hachi bu), turn away from anger a bit more, exercise with fun is more a benefit to our longevity than all the crap we are blasted with.

Imagine (ala John Lenon), imagine if we all are just nice.

Most of the time, a dental abscess is due to decay forming and the combination of saliva, dark environment, sugars and inefficient cleaning, makes the oral bacteria, very happy. So happy, that they use the tooth as a growth medium and will migrate and multiply into the hard tissue of the tooth and change its consistency. The tooth now becomes soft where the caries form and eventually these little bacterial bu**ers, will migrate into the well-hidden pulp chambre of the tooth.

Now, all hell will brake loose, as the goggos pe*****te the pulp and start causing pain, which is the start of an abscess. Decay can not be stopped by anything. It has to be removed and cleaned out, so antibiotics are now taken orally and via your bloodstream, reach the pulp of the guilty tooth, with some positive effect. The thing is, that the decay needs to be removed and by now , a root canal treatment might be needed. If you wait too long, your body will scream at you and will protect itself against the apex of the tooth by encapsulating the apex with epithelium. It is now, that the effectivity of anti-biotics might not be that good any more.

So, if decay forms, see a dentist. If you wait too long you might sit with the problem on Christmas morning. Take the anti-biotic but please also have the tooth seen to during the course of the anti-biotics.

Why antibiotics somethimes do not work?

Most of the time, an abscess of dental origin, is due to decay forming and the combination of dark environment in the mouth, with salvia and carbohydrates, makes the oral bacteria extremely happy. So happy, that they use the tooth as a growth medium and will migrate and multiply, into the hard dental tissue and change its consistency. Gradually, these bugs will, eventually migrate into the pulp chamber of the tooth.

It is now that all hell breaks loose. Decay can not be stopped after it started. It can only be removed. So, now, pain sets in and an antibiotic is prescribed that is taken orally, and via your blood, arrives at the crime site.

Initially it will work but there needs to be work done on the tooth also, as this is the cause. An antibiotic on its own, is not good enough.

There comes a time, where your body “encapsulates” the apex of that tooth. This now means that the antibiotics in your blood, can not reach the cause of the abscess… The large decayed area of the tooth.

Take the antibiotics, but please let a dentist take care of the cause also during the course of the antibiotic.

With modern local anesthetics, risk to unwanted reactions is very low, providing you stay well within prescribed volumes injected to a patient, as indicated per kg of the person.

We often hear a patient remark that they are allergic to adrenaline. This is not possible as your own body provides you daily with lots of adrenaline from adrenal glands on top of your kidneys. Allergy can only from other components in head health. A bit of heart palpitation as local anesthetic is applied, is quite normal. The best way to counter this, is to have something to eat before having the local anesthetic.

Local anesthetic in dentistry, with correct application, is LOCAL. The raw anesthetic in it is safer and allows for gradual breakdown by your body. A child under 12 years, can, in any case, not receive non-vaso considered local anesthetic.

Brushing your teeth!

Many years ago, a colleque of mine, Dr. Erasmus, did electron microscoping studies of “the best toothbrush” brand, and found their claim to fame, very wrong. The shape of a brush’s hair, has very little to do with the efficiency of cleaning your teeth.

Scrubbing your teeth sideways, has the same effect as a belt sander on your teeth. It takes time to prove. Circular movements with a toothbrush, is equally ineffective. Up and down movements is only good when skipping rope in the gym. There is only ONE proper way to brush, and it is the stillman’s technique. It is the only way, regardless what companies show in TV adverts. Nothing else is effective, no matter which toothbrush or what toothpaste.

Google it and apply it, with daily flossing. I will say again… Nothing else works, unless you feel comfortable with the belt sander in your mouth. Problem is, it is a bit hard on your lips also, so try Stillman’s and floss.

Oh, and by the way, try bamboo brushes and let’s move away from plastic. Fish will love you for that.

Why a Crown?

Some work need a filling due to decay, but sometimes this decay is too big to handle chewing forces.

The same happens with a tooth that had a root canal treatment. These large fillings tend to fracture and then a crown/cap is the solution.

A crown works like a thimble, that encapsulates the whole visible part of the tooth. This “ties” the tooth together and gives strength to the tooth.

Are they flexible? Of course they are. The cement layer should be between 15-40 microns thick and the person who does the chewing, must be aware of that.

This thimble needs good care with flossing and some level of respect when biting hard and sticky stuff.

Crowns can last for 20 plus years, but need maintenance and respect!

Deciduous teeth really have a bad adult card!

They are temporary, need to maintain space for permanent teeth, they decay easily and are exchanged for money when they are not usable any more.

Regardless, it is vitally important to keep them pleased (every day) and decay free. The problem is that children can not floss. Adults must do that for the child.

Decay form easily due to this and a bit of decay can, within 6 months, turn into an abscess. This is then treated with a pulpotomy, which is a half witted adult tooth root canal.

Pulpotomy is done because the primary tooth only has a limited, very short lifetime. It will be pushed out by the permanent tooth in good time.

The important parts are to help the adult abscess heal and keep space for the permanent, soon to follow, tooth. Remember, also, to not involve the Toothmouse and Father Christmas in your child’s education.
