A poorly aesthetic crown is rarely just a crown problem.

This patient came to us unhappy with the final restoration. What she was actually living with was an implant in the wrong three-dimensional position,soft tissue that had never been developed, and a crown built short to try and hide both.

When we turned the screw to remove the crown, the implant turned with it. It had never integrated. So it came out, and we started again properly.

Four months of healing. Then a fully guided rebuild: the implant position planned in 3D, the surgical guide and the provisional both made before the patient sat in the chair. Bone reduced to create the EBC space, the buccal wall contoured to give the soft tissue somewhere to go, and a modified infold
technique to develop the profile.

The last slide is day one. Immediate provisional in place, soft tissue sutured around it, emergence profile and colour match by the guys at Diceram. The definitive crown follows once the tissue has matured, and we will post it.

A little of our own history, this one felt worth sharing.

IAA was born from a conversation between two friends who believed South African clinicians deserved better.

It started on a train in Europe — Prof Howard Gluckman, Dr Mark Otto, and Professor Andre Van Zyl, talking about what dental education in South Africa was missing. Our first course crammed eight students into a small room at Mark’s own practice. Twenty-two years later, every course we run still carries a piece of what those two friends started together.
