How’s that for some serious treatment planning whilst a fish is waiting to be caught on the Chobe river- at my front door- multiple implant planning for an edentulous patient with guided surgery.

There’s no better alternative than digital dentistry planned and executed with preemptive restorative designs.

A classic example of lingual cusps taking a hammering under loads. Previous amalgam filling with liner; of course!

I considered simply doing two direct restorations into the buccal fissure areas where there was insidious decay, but in the end, decided to include these into the ceramic. Buccal cusps did not warrant overlaying. Managing the mesial contact was probably the most challenging part. Fortunately, my lab FK Dental Laboratory and I have started doing AnyDesk discussions about the areas of the preparation I was unsure of whether they would be happy with or could work with.

My hesitation was related to unnecessarily extending the MB line angle by flaring the enamel here for the sake of opening the mesial aspect for a full ceramic contact... and potentially weakening the MB cusp; Removing sound tooth structure
❌ CRE did not dictate further extension; so, half of the contact is ceramic the other half natural.

Created space with the Ivory Separator to do minimal invasive preparation on the mesial along with the use of the Komet ultrasonic preparation burs in air scaler.

I always try and have a conversation with the patient afterward in reflecting on the photographs about my intra-operative concerns in trying something. There is an opposing composite with a large isthmus that may be on the cards for revision. It helps when your patient is lovely 😇
