Double trouble on 15 & 16.

The 16 was a retreatment with short previous obturation and a leaking crown. Regaining patency was definitely the biggest challenge here, but we got there. Retreatment completed with BlueShaper PRO.

The 15 had some beautiful S-shaped anatomy and Excalibur PRO Mini made the prep quick and controlled.

5,25% hypo, 17% EDTA, U-files and sonic activation.

Deep margins managed with the DME Wedge and G-ænial Universal Injectable, followed by EverX core build-ups.

The 16 was temporised with a fully bonded bis-acryl crown and referred back for the definitive indirect restoration immediately.

I wish I had documented more of this case, but once the rubber dam went on, it was all cylinders go 💨

Not every J-shaped radiolucency is a vertical root fracture. 🦷

Yes, it’s a classic radiographic presentation but it’s not pathognomonic for a VRF.

Endodontic infection and other endo-perio pathology can produce very similar patterns of bone loss.

Before condemning the tooth, correlate the radiographic appearance with the clinical findings, probing, history and CBCT.

A few days ago, I posted a story saying “stop asking me what files I use, start asking me how I managed the case.”

It got a lot of reactions.

So here’s the case behind that statement: a cracked, molarised premolar with a necrotic pulp and symptomatic apical periodontitis.

37 referred due to difficulty navigating a significantly curved distal canal.

Careful negotiation, a reproducible glide path and controlled shaping allowed us to safely follow the original canal anatomy to length.
