WHEN COMPLEX PROBLEMS REQUIRE MULTIDISCIPLINARY SOLUTIONS

Multidisciplinary collaboration has always been one of the principles I value most, because some medical challenges are simply too complex for a single specialty to solve alone.

This patient had a particularly rare condition: the right TMJ was ankylosed despite previous surgery, while the left TMJ was congenitally absent. The result was severe mandibular deficiency, a critically narrowed airway, loud snoring, and very severe obstructive sleep apnea (AHI ~60), despite a normal body weight.

The treatment plan involved advancing the jaws to enlarge the airway and reconstructing both TMJs. However, stock prostheses were not suitable due to the patient’s unique anatomy and the planned skeletal movements.

Fortunately, together with the VinUni 3D Team, we were able to design and manufacture a patient-specific bilateral TMJ prosthesis tailored to this case.

After nearly 10 hours of surgery, the patient has:
• Gained 8 kg and improved chewing function.
• Completely resolved snoring and sleep-related breathing difficulties.
• Achieved a much more balanced facial appearance.

LE FORT I + II + III: RECONSTRUCTING THE ENTIRE MIDFACE

This patient had undergone preoperative orthodontic treatment in preparation for corrective jaw surgery for skeletal Class III malocclusion. However, clinical examination and 3D analysis revealed that the problem was not limited to the tooth-bearing maxilla (Le Fort I segment) and the mandible.

The entire midface, including the zygomas, infraorbital rims, and nasal bone, was underdeveloped and positioned posteriorly. If conventional orthognathic surgery had been performed using a standard Le Fort I advancement combined with mandibular setback (BSSO), the occlusion could have been corrected, but the aesthetic outcome would likely have been suboptimal and might even have accentuated the midface deficiency.

In addition, the central facial unit (the nasomaxillary complex) was also retruded relative to the zygomatic prominences, contributing to the overall facial disharmony.

After comprehensive evaluation, the treatment plan involved independent mobilization and advancement of three distinct anatomical units of the midface:

• Nasomaxillary complex (Le Fort II)

• Bilateral zygomatic complexes (Le Fort III)

• Tooth-bearing maxilla (Le Fort I)

Each segment was advanced by a different amount to restore a more appropriate skeletal relationship and achieve balanced facial proportions. Simultaneously, mandibular setback surgery and genioplasty were performed to optimize the overall facial profile and skeletal harmony.

The current outcome demonstrates significant improvement in midfacial projection, enhanced facial balance, and a stable, well-interdigitated occlusion. Most importantly, the patient has recovered well and is now able to confidently enjoy one of the most memorable milestones of her youth — her graduation photographs.
