Preparing for your first visit to our oral surgery practice? Here's what to bring:

Surgical referral slip and any x-rays (if applicable)

A list of medications you're currently taking and your doctor’s contact info

Medical and dental insurance cards

If you're under 18, a parent or guardian must accompany you. If surgery is needed, please fast for 8 hours beforehand and arrange for a ride home.
