Case of the week: "An oral surgeon's nightmare."

The patient is a 24 year old female. She was told several times by her Dentist to not worry about her wisdom teeth. She presented to my office with the complaint of pain in the bottom left. A x-ray showed an extra wisdom tooth on the top left (red), fully grown wisdom teeth on the bottom (red) with roots extending past the inferior alveolar nerves (yellow) to the bottom of the lower jaw. So 5 wisdom teeth. A CBCT scan showed a nerve canal transposition meaning it was located on the cheek side of the mandible and not the tongue side. We cut bone on the cheek side so nerve injury potential was very high. A very serious discussion about permanent nerve injury was discussed in detail as was the possibility of a fractured jaw because there was no bone south of the lower wisdom teeth root tip (arrows). This could all have been avoided by seeing an oral surgeon when she was 16. The surgery was very delicate and careful taking 3 times my normal operating time. Thankfully there were no complications and the patient healed remarkably well. Please see your oral surgeon for wisdom teeth evaluation sooner rather than later.

This is a lot like my olive oil we sell... if you don't know, you don't know.

A 79 year old female who has been a patient forever presented with an implant crown and screw from an implant #30 I placed in 2024. Her dentist in Maryland restored the implant but used a lab screw made of steel instead of the manufacturers recommended gold screw that deforms under torquing pressure and metal welds the connection whereas steel does not deform since its harder and does not weld. So, lab screws will loosen frequently. Today I replaced the crown with a company genuine gold screw that should not loosen. When gold costs $4500 an ounce dentists will use cheaper alternatives to lessen costs. Like olive oil. If you don't know a guy who imports the finest product that is the real thing then you are using fake oil, like a fake implant screw.

THIS IS WHY YOU GO TO YOUR DENTIST TWICE A YEAR!!!
A 23 year old healthy female pre-school teacher was referred to evaluate and treat a white spot on her left lateral tongue. There was no history of to***co use or alcohol abuse. An excisional biopsy removed the lesion and under the microscope showed SEVERE dysplasia. When tissue is abnormal there are 4 stages before it becomes cancer. Mild, Moderate, Severe and Carcinoma in Situ. This tiny, innocuous 1mm white spot blew through 3 stages and was headed for life changing interventions at just age 23. The biopsy should be curative and the patient will be followed very closely for the rest of her life by her Dentist, Me, ENT and OB/GYN where there same tissue grows.

A healthy 23 year old woman presented for removal of 2 upper wisdom teeth. The surgery was uneventful. Postoperatively the patient called complaining of a bloody nose and persistent bleeding beyond the usual 3 or 4 hours. The bleeding stopped later in the evening. One week post op the patient presented for normal follow up exam and demonstrated infra orbital ecchymosis, or bruising, as well as bilateral cheek bruising. On further questioning, the patient admitted to heavy me**es and recalled a blood workup when she was a child but no diagnosis of an abnormality was made. I ordered clotting tests and discovered a prolonged PTT. The patient was referred to a Hematologist who conducted more extensive bleeding disorder tests. A Factor deficiency in the clotting cascade was discovered that will require management should the patient have surgery in the future or sustain heavy bleeding from a trauma.

Don't let this be your child at age 82! High High High risk of a mandible fracture and permanent nerve injury removing this infected lower wisdom tooth.

Always seek out an oral and maxillofacial surgeon when your children are in high school for evaluation and treatment of wisdom teeth. It doesnt HAVE to be a miserable experience in the right hands!

Case of the Week:
We go to Cortina and Lindsay Vonn this week. The erector set that was placed on the patient's leg stabilized a complex fracture with many pieces of bone. In oral and maxillofacial surgery we use the same technique when fractures are complicated and cutting risks compromise of blood supply leading to dead bone or bone that is simply missing from a bullet or blast injury. Pins are placed through the skin above and below the fracture then the pins are stabilized with external metal framework to immobilize the fracture. The fracture can then be definitively treated with plates and screws at a later date. Dr. Hacket in Vail, CO helped me with a complex clavicle fracture I sustained from a skiing accident 2 years ago and has been by Lindsay's side since tearing her ACL 2 weeks ago. Today her leg was opend and definitively treated with long titanium plates and screws. Praying for no infections after multiple procedures. World class operators doing miracle surgery saving a warrior's leg on two continents.
