The AAO published it, and thank you to Dr. Michael DeLuke for analyzing it. I’m here to give my take because I do agree with Dr. DeLuke’s take on this. 👇

Here’s the good, the bad, and the ugly of this AAO White Paper on Airway and Sleep Disordered Breathing.

✅ GOOD — Acknowledges airway problems exist on a spectrum and encourages interdisciplinary screening. Progress.
⚠️ BAD — Panel had almost no outside experts. Barely touched on nasal breathing or how to spot airway dysfunction in kids.
🚨 UGLY — Requires a sleep study confirmed sleep apnea before intervention — even though many kids with clear airway problems still pass their sleep study. Also claims mouth breathing can’t be identified, kids will outgrow it, and CBCT has no diagnostic value.

TMJ flare-ups don’t always have to follow you on your trip. Here’s your jaw-friendly travel guide:

❌ What NOT to do:
∙ Skip packing your Orthotic
∙ Eat hard, crunchy, or chewy foods on the go
∙ Hunch over your phone or laptop for hours
∙ Chew gum to pass the time
∙ Overpack your schedule and skip rest time
∙ Let dehydration sneak up on you

✅ What TO do:
∙ Bring your neck pillow, Orthotic + heat/cold packs
∙ Choose and prep soft, easy-to-chew foods/snacks
∙ Keep your head stacked over your shoulders and use neck support
∙ Stand, stretch, and move whenever you can
∙ Build rest into your itinerary
∙ Stay hydrated and practice deep breathing

Your jaw works hard every day, give it a little extra support when you travel 🙏

Summer get-togethers are here, and that means many food options!

Here’s what to skip if you have jaw pain👇
❌ Hard, crusty breads — bagels, French bread
❌ Crunchy foods — granola, nuts
❌ Tough meats — steak, beef jerky
❌ Raw salads & crunchy fruits or veggies
❌ Caffeine — it can increase muscle tension and make flare-ups worse

Here’s how to keep your nutrition up without stressing your jaw 👇
🥤 Fruit smoothies (add spinach or kale for extra greens)
🍝 Soft bread, rice & pasta for fiber and fullness
🐟 Fish, eggs, Greek yogurt & cottage cheese for protein
🥦 Cook your veggies until they’re easy to chew

The goal: nutritious AND easy to chew. That’s it!
And remember — this is temporary. Once your pain is under control, you can slowly bring back your favorite foods.

I wanted to take a second to correct the information I spoke about the other day in a video around Masseter Botox and what I thought was muscle atrophy happening. I intend to never mislead you, but instead provide the best information I can as a TMJ Dentist.

Having said that, please keep in mind that I do not use Botox as a first line or long-term treatment for TMJ/TMD because it is simply not appropriate. This is why I did not recognize paradoxical bulging.

If you are using Botox as a long-term solution and doing it every 3 to 4 months for your patients, I strongly believe you’re doing them a huge disservice.

Additionally, I think it’s important to be kind on the Internet. It is very easy to make broad statements and say things like: “ you’re spreading misinformation” (like I’m saying incorrect things purposely. 🙄) and “make sure your information is evidence-based” when truthfully speaking, the evidence around Botox for treating TMD is not great.

NOW - Let’s get into the meat of it. Botox is a Neurotoxin. When injected into the Masseter muscles, it paralyzes those muscles, and they essentially “stop functioning,” to reduce clenching activity and reduce pain.

Two potential side effects of Botox:

💉 1) Paradoxical Bulging: when there is an uneven distribution of Botox into the muscle fibers.

💉 2) Muscle Atrophy. This occurs after Botox is injected multiple times - every 3-4 months for about a year. This can cause the masseter muscle to atrophy, weaken, and get significantly smaller. This will change the shape of the face AND affect the size of the airway when sleeping AND cannot be easily reversed.

As a TMJ Dentist, Botox is not my first line of treatment for TMJ/TMD; I do not recommend it. It isn’t treating the root problem; it’s only paralyzing it.

Last week, we discussed Orofacial Pain vs Neuromuscular Dentistry, but I was reminded that I missed a 3rd camp! So let’s discuss 🗣️

The 3rd camp is “Joint Forward.” This is all about positioning the condyle (the little bony k**b that the jawbone ends on) in the jaw joint.

Again, not here to start a debate or crown a “winner” in the TMJ world… I truly just like to share what I’ve seen work for my patients, and staying open to learning along the way and having fun discussions!
