Facial harmony. A balanced bite. Confidence in every smile. ✨

This young man’s journey began with extensive orthodontics and treatment to guide jaw growth. Despite those early efforts, a difference in his upper and lower jaw development persisted.

Working closely with his orthodontist, we planned corrective jaw surgery with connected goals: improving bite alignment and chewing function, bringing greater harmony to his facial proportions and profile, and accounting for airway health and breathing in the surgical plan.

These photos share one part of that journey. What means the most to us is the opportunity to help patients feel more comfortable and confident in themselves while caring for their long-term function.

A narrow upper jaw can affect tooth alignment, bite relationships, and the nasal airway.

Miniscrew-assisted palatal expansion uses small, temporary bi-cortical anchors in palatal bone to help skeletal widening of the upper jaw. For selected patients, expansion may also increase nasal airway dimensions and improve nasal breathing.

MASPE or MARPE?
Slow and rapid expansion protocols offer different approaches. Appliance design, activation speed, and coordination with braces or aligners are tailored to each patient and directed by the orthodontist.

Both MASPE & MARPE appears to have reduced the incidence of formal corrective jaw surgery in transverse dimensional deficits.

At Inland Institute, we collaborate with your orthodontist to address jaw width, bite function, and opportunities for airway optimization.

Breathing benefits vary. Expansion alone is not a guaranteed treatment for snoring or obstructive sleep apnea; those concerns require a comprehensive evaluation.

🦷 Oral Surgery 101: Two jaw fractures. Two different treatments. One goal—restore her bite and function.

A fall at home left this young mother with two fractures of her lower jaw. Her care illustrates an important principle: each fracture deserves its own treatment plan.

🔹 One fracture required ORIF: open reduction and internal fixation—carefully realigning the bone and securing it with a plate and screws.

🔹 The other was treated with closed reduction and IMF: aligning the jaw without surgically exposing that fracture, then using fixation between the upper and lower teeth to support healing.

How do we choose the approach?

• Where the fracture is located
• How far the bone has shifted and how stable it is
• Whether the normal bite can be restored and maintained
• The condition of the teeth, bone, and surrounding tissues
• The patient’s health and ability to tolerate fixation and follow recovery instructions

Our aim is the least invasive approach that provides reliable alignment and healing. When appropriate, incisions can be placed inside the mouth, avoiding visible facial scars. Closed treatment can also spare selected fractures from surgical exposure.

Oral and maxillofacial surgery extends well beyond wisdom teeth. Caring for facial injuries means helping people regain the everyday functions we often take for granted—eating, speaking, and smiling.

🔴 What in the world are those red spots?

This patient underwent corrective jaw surgery using Invisalign®—without conventional braces.

The red spots mark where temporary anchorage devices (TADs) were recently removed. These small surgical anchors help position and stabilize the jaws during surgery and can support postoperative elastics to guide the bite during healing.

He will have some minor post surgical orthodontic detailing to complete his treatment.

Small tools. Precise control. A beautifully aligned result.

When front teeth are traumatically knocked out, the injury often extends far beyond the teeth themselves.

Traumatic avulsion can also result in significant loss of the surrounding gum tissue and alveolar bone—the very foundation needed for future dental implants.

For an 18-year-old young woman, suddenly losing teeth #8 and #9 can be emotionally overwhelming. Restoring her smile requires not only replacing the missing teeth, but carefully rebuilding what was lost beneath them.

In this case, bone harvested from her wisdom-tooth extraction sites was combined with donor bone to reconstruct the deficient ridge using bone grafting and guided bone regeneration.

We will now allow approximately four months for healing and maturation. With careful planning, favorable healing, and a little blessing from above, we hope to place dental implants that will permanently restore her front teeth—and, most importantly, help restore her confidence.

Reconstruction is rarely about replacing teeth alone. It is about restoring function, appearance, and the person behind the smile.

Innovation is exciting—but good treatment planning is timeless.

In today’s dental implant world, we often see remarkable techniques using remote anchorage—including zygomatic, pterygoid, and strategically angled implants. These approaches can be invaluable when anatomy and clinical circumstances require them.

However, newer does not always mean better for every patient. Traditional implant and grafting techniques remain predictable, biologically sound, and highly effective when thoughtfully selected—as demonstrated in this case.

Even after many years in surgery, I remain amazed by what collaboration and digital technology allow us to accomplish: an iPad, gifted restorative dentists/prosthodontists, talented laboratory technicians, skilled digital designers, and precise 3D printing—all working together to transform a carefully developed plan into a functional, natural result.

The goal is never simply to use the newest technique. It is to select the safest, most predictable, least invasive and most appropriate treatment for the individual patient.

Grateful for the remarkable teams and technology that continue to elevate what is possible in implant reconstruction.
