Resection: Took down the mesial root ends of tooth 30 cleanly.
Prep & Seal: Thorough retroprep followed by a flawless bioceramic seal. 🧱
Regen: Placed a bone graft to kickstart healing. 🦴
Closure: Sutured up perfectly while keeping the original crown completely untouched. 👑

Next Steps 🗓️
The patient is healing smoothly and will be back in exactly one week for suture removal. After that, we transition to long-term radiographic monitoring to watch that bone fill in! 🕵️‍♂️

Saving the “Unsaveable” 🦷
Today’s case was a true test of persistence. Tooth #30 presented with persistent swelling and a mid-root perforation—a scenario that often leads to extraction.
After discussing the guarded prognosis, the patient chose to fight for their natural tooth. Our approach:
✅ Nonsurgical retreatment
✅ MTA repair of the perforation
✅ Bonded core build-up to seal the deal
We’re keeping the existing crown for now and monitoring closely. If the fistula resolves in the next two weeks, it’s a win for tooth preservation! - Completed by Dr Nugent

#30 came in previously endo treated (shoutout to the in-house endo 👀). Not one, not two… but lesions around each root end — plus a bonus lateral lesion halfway down the mesial root just to keep things interesting.

Retreatment through the crown, 5 canals obturated… and yes, we went fishing for that middle mesial canal — because if you don’t look, you won’t find it. 🎯

Fast forward to follow-up: apical healing is evident and that lateral lesion has officially packed its bags and left.

Clinical win (for now)… and definitely worth the blood, sweat, and tears.

PS — glad we replaced the crown too… whew. 😅
