🩷 Months of Mouth Ulcers? Don’t Just Call Them Canker Sores.

This patient had painful ulcerations throughout the mouth for months, making eating and everyday life difficult. The eventual diagnosis was Pemphigus Vulgaris (PV)—a rare autoimmune disease that causes the cells lining the mouth to lose their normal connections, resulting in fragile blisters and erosions.
But PV can look like several other conditions, including other causes of desquamative gingivitis and autoimmune blistering diseases. That’s why the first step is getting the diagnosis right.
🔬 Step 1: Biopsy
A biopsy, including tissue for direct immunofluorescence, is important to confirm PV and distinguish it from other conditions. Blood antibody testing may also help.
💊 Step 2: Proper medical management
Once confirmed, management is typically coordinated with a dermatologist/oral medicine specialist. Treatment may include corticosteroids and other immune-modulating medications such as rituximab, depending on disease severity and the individual patient. Oral care, pain control, nutrition, and treatment of secondary infections are also important.

The takeaway:
👄 Persistent oral ulcers that last for weeks or months deserve an evaluation—not just another tube of ulcer medication.

Sometimes the mouth is the first place a systemic disease reveals itself.

🦷 Trauma. A 5mm Pocket. Then… Regeneration.
This tooth suffered trauma in March 2025. Even after stabilization and endodontic evaluation, it still had a 5mm pocket, bleeding, and swelling 6 months later.
We suspected a localized bone defect and performed Guided Tissue Regeneration (GTR).

So, what is GTR?
GTR is a regenerative procedure that uses a barrier membrane to protect a bone defect and create the space and environment needed for the body to rebuild lost periodontal supporting tissues. 🦴🌱

9 months after the regeneration procedure was performed:
✨ 5mm → 3mm pocket
🩸 BOP → None
🦴 Stable radiographic bone level
🌱 Healthy gum tissue

Sometimes saving a traumatized tooth means treating the foundation—not just the tooth.

🦷 Sometimes Implants Need More Than Bone — They Need Strong, Attached Gum
When an implant is planned in an edentulous ridge, we often think first about having enough bone.
But bone is only part of the foundation.
For this patient, the plan was an implant-supported overdenture using two implants. The ridge had inadequate attached/keratinized gum tissue around the planned implant sites, so a free gingival graft (FGG) was performed to create a wider zone of firm, attached tissue.
🌱 Why does that matter?
A healthy band of attached gum can help:
- Protect the tissue around the implants
- Make the area easier to keep clean
- Improve soft-tissue stability around the implant
- Provide a healthier environment for long-term implant maintenance
The goal wasn't to make the ridge look different just for appearance.
It was about creating the right soft-tissue foundation for the implants and the overdenture that would follow.

Two implants. One overdenture. A stronger foundation.

Because implant treatment isn't just about placing the implant—it's about preparing the tissues around it for what comes next. 🌱

🦷 When There Isn’t Enough Bone… Don’t Just “Shovel in” the Implant

An implant is only as good as the foundation around it.
When the bone ridge is too narrow or there simply isn’t enough bone, placing an implant without addressing the deficiency can lead to:
- Implant exposed through the bone
- Gum recession
- Loss of bone around the implant
- Thin or fragile gum tissue
- Unfavorable esthetics
- More difficult to keep clean
- More complicated treatment later (= More 💸💸💸💸💸 later)
So instead of simply “putting the implant where the bone is,” sometimes we need to build the bone first.

Build the foundation. Then build the smile.

In this case, Guided Bone Regeneration (GBR) was used to transform a narrow, inadequate ridge into a fuller, adequate foundation for future implant placement.
Narrow ridge → GBR → Full ridge → Implant
Because sometimes the most important part of implant treatment happens before the implant is placed. 🌱

🦷 Short Teeth? Sometimes, It’s the Gum & Bone!
When teeth appear too short, the problem isn’t always the size of the teeth. Sometimes, too much gum and supporting bone are covering the natural tooth structure.
✨ Esthetic crown lengthening carefully reshapes the gum and, when indicated, the underlying bone to reveal more of the natural tooth.
Think of it as creating the right balance between:
🦷 Tooth — how much natural tooth is visible
🌱 Gum — where the gum margin sits
🦴 Bone — where the supporting bone needs to be positioned so the gum can heal in a stable, natural location
The goal isn’t simply to “remove gum.” It’s to re-establish the proper relationship between bone, gum, and tooth to create a longer, more balanced and natural-looking smile.
Sometimes, the tooth isn’t too short.
It’s just hiding! 😉

🦷 Don’t Just Chase the Gum Line—Treat the Foundation.
When gum recession exposes the roots, simply trying to move the gum line higher without addressing the underlying tissue condition may result in further recession or an even lower gum line over time.
In this connective tissue grafting case, the goal wasn’t just to cover the exposed root. It was to transform thin, fragile, non-attached tissue into a thicker, firmer, and more stable band of attached keratinized gum. 🌱
✨ The result:
A healthier protective boundary + improved tissue stability + a more harmonious gum line.
Sometimes, the best esthetic result comes from building healthy tissue first—not simply chasing the gum margin.

🦷 Can a Problematic Implant Be Saved? Sometimes, Yes.
This case is a great example of why early detection and thoughtful periodontal treatment can make a difference.
The patient had two lateral incisor implants placed and restored about a year ago. A referring dentist noticed gum recession around both implants, a collapsed appearance from the front, and a fenestration with minor suppuration around one implant.

The patient came to us for a second opinion, hoping to avoid removing the implants and starting everything over.

Fortunately, there were some encouraging factors: the surrounding interproximal bone levels were still relatively well maintained, and the remaining bone provided a reasonable foundation to work with. The fenestration and suppuration did make the prognosis more challenging—but not hopeless.

🌱 Bone sets the tone, but soft tissue can change the game.

By using a specialized gum grafting approach, we were able to increase tissue thickness and create a firmer, tighter soft-tissue seal around the implants to better protect the remaining bone.

✨ At just 28 days:
• Gum recession corrected to the maximum level allowed by the restoration’s emergence profile
• Gum thickness significantly improved
• The fenestration was closed
• The implants remain in place

This is still an evolving case, and we’re excited to watch the tissue continue to mature.

Early detection matters. If you notice recession, inflammation, or changes around an implant, getting it evaluated early may give you more treatment options.
