🛡️ Building a Strong Foundation: Rebuilding the Gum Barrier
When it comes to gum health, it’s not just about aesthetics—it’s about structural defense. This patient came to Origin Periodontics feeling a bit nervous, especially concerning a low frenum tie (the muscle attachment) between their teeth and wanting to prevent further recession.
🔍 The Challenge & Expectations
Due to anatomical and biological limitations—specifically pre-existing bone loss on the side of one tooth—complete root coverage wasn't biologically possible. Before surgery, we set a realistic expectation: a maximum coverage of 3mm below the tip of the papilla.
Our primary goal wasn't a cosmetic quick-fix; it was to restore the missing attached keratinized gingiva (the firm, protective gum barrier) to stop further recession in its tracks.
🔬 The Technique
To address the patient's concerns about the frenum tie, we utilized a specialized approach that required no cuts to the frenum itself. Instead, the tie was gently released underneath the top layer of tissue during the connective tissue graft procedure, protecting the anatomy while relieving the tension pulling on the gums.

A Widened Defense: The patient now has a significantly wider and thicker bank of attached, protective gingiva.
Successful Coverage: Gums were successfully covered right up to our pre-op biological expectation.
