Precision, Patience, Perfection. That’s what interdisciplinary treatment is all about! ✨🦷

This 48-year-old female presented with significant challenges: a compromised extraction of tooth #9, vertical and horizontal bone loss, and an edentulous space wider than tooth #8. Here’s how teamwork and phased treatment created a beautiful, functional smile:

➡️ Hard tissue augmentation ( #8) using titanium mesh
➡️ Connective tissue augmentation (facial #10, #11)
➡️ 9-month hard tissue maturation and implant placement with additional soft tissue augmentation
➡️ Orthodontic eruption of tooth #10 to optimize papilla formation
➡️ Composite build-up on tooth #10 for ideal spacing
➡️ Veneer preparation and provisionalization of maxillary incisors
➡️ Final screw-retained implant restoration (custom abutment + Emax crown)
➡️ Emax veneers for the finishing touch

Imagine dealing with a lifetime of dentistry due to a congenital birth defect…

This cleft lip-palate patient is just that.

In this late stage adult having multiple full restorations done in his lifetime, where do you start….cleft repair? Failing dentition?

How about both! Transitioned the failing dentition with selective extraction and implant placement. Teeth #’s 5 and 9 were root banked to keep the bone and make the cleft repair easier. Multi-base abutments were used. Custom abutments were picked up intra-orally with resin cement allowing a completely passive fit screw retained zirconia superstructure.

Mount Rainier National Park - a must visit if you come visit Washington state! This United States National Park located only 2 hours out of Seattle was our countries 5th ever recognized National Park! The entire park encompasses 236,281 acres including stratovolcano Mount Rainier, the highest point in the Cascades at 14,411 ft!

No simple fix ⚒️
Patient presents with a previously failed implant in #8 site.

The resultant hard and soft tissue defect upon implant removal can be clearly seen in the #8 site (as well as the attachment loss on the mesial/facial of #7).
Although the use of orthodontic eruption of #7 was discussed as a possible adjunct, her treatment only included surgical augmentation (hard and soft tissue). The #8 implant was provisionalized to help contour the soft tissue and support papillae prior to composite placement on the mesial of #7 to close the remaining black triangle.

Definitive restorations consisted of a single tooth implant #8 and a ceramic veneer #9.

For 27 years Suite 490 has been our work home. It was the birthplace of Dr Frank Spear’s Seattle Institute for Advanced Dental Education which evolved into what we now know as Spear Education. Great memories…great dentistry.

Thank you 600 Broadway… it’s been fun.
