Small teeth combined with spaces between them is a common situation. It is usually easily corrected with porcelain veneers, because only little tooth structure needs to be removed to create space for the porcelain. However, special consideration needs to be given to a worn dentition. Any future restoration of worn teeth needs to be in harmony with the home position of the temporomandibular joints as well as the customary functional movements of the patient's mandible against the maxilla.

What evokes in us the recognition of something that pleases our aesthetic senses?

Yes, beauty is in the eye of the beholder and doesn't exist on its own, but is created by each one of us as observers.

We often talk about "beautiful smiles." This often distracts from the contributing factors that allure us to indulge beauty.

What do you see here? To me, it is a proclamation that beauty is not a static perfection. It is rather a composition of harmony with the footprints of life: This mouth has lived!

It declares persistence and determination, with signs of battle, yet with a beauty that is fluid and fully in sync with life. ♥️

The last 18 months have been challenging on so many levels, but our deepest levels of human compassion, friendship, and affection have been tested the most. They often went missing and many of us endured a feeling of great loss.

But we can still help each other. As soon as someone sets a goal, teamwork becomes necessary to achieve it, with compassion, understanding, and the desire to help each other. This often makes up for the big loss in our personal lives.

My contribution towards a greater happiness in my community is cosmetic dentistry. I use my training and experience as best as I can to help my patients to reach their goal to feeling better about themselves.

My contribution is only a small token in the big picture of things. At the end, all of us together, having devoted our lives towards helping in a profession that we felt compelled to exercise, can create a happier community for each other.

The smile below is one small token of my help. It was possible only with the help of various experts, each of whom contributed with his/her experience and expertise.

The patient has no lateral incisors and presented with existing crowns on the canines, attempting to approach the shape of lateral incisors. The patient decided to improve the current appearance of her smile.

Many years ago I came to this country as a post-doctoral fellow to do AIDS research at UCSF. During the completion of my PhD thesis in Berlin, Germany, it had become already apparent that infection control had to be raised to an unprecedented level. HIV is a blood-borne virus that forced especially dentists to practice stringent infection control of all surfaces.

COVID-19 is airborne.

It appears that many people have still not understood the ramifications of an airborne pathogen. It floats in the air.

Dentistry has historically been the profession with the generally highest level of infection control, surpassed only by the sterile environments of operating rooms in hospitals.

This new virus disrupts our social welfare and cultural well-being. I understand the primal instinct to wanting to "revolt" against this invisible enemy or even to ignore it. However, this would not just be unwise on an individual basis, it would be reckless regarding the well-being of everyone around us.

All of us have to take responsibility. I have to do my part at the highest level. My patients entrust their health into my hands. They are entirely defenseless when they have to take off their masks and open their mouths.

"Airborne" means what it says: transported by air. All of us need to fully assimilate this.

COVID-19 is not a blood-borne pathogen, it is transported by air. It is only 0.1 micron ( = 100 nm) in diameter. Light dust particles are 1.0 micron in diameter and cause allergies. The naked-eye visibility threshold is 40 micron.

Integral part of my airborne-pathogen adapted infection-control protocol is the highest possible separation of breathing air between my patient and myself.

I only see one patient per day. The treatment room is radiated with UV-C light prior the occupation of the room. High-volume HEPA filter units are constantly running to filter out any particles from the air.

Every patients will have completed a COVID-19 saliva test within a few dsys days of the appointment with self-isolation after collection of the saliva sample. I myself have been practicing extreme self-isolation and have had four COVID-19 tests, all of them demonstrating that I have not been exposed to the virus.

While COVID-19 disrupts our social interaction and culture severely, I believe that the dental office should continue to be one of the safest locations on our planet.

I have to mention once in a while the organization that has helped me reach the level of skills and experience that I have today.
It cannot be overstated how much inspiration and direction towards the highest standard of excellence the American Academy of Cosmetic Dentistry (AACD) has given to me.
It is not just an academic organization that hands out memberships for a tuition, in fact, it is the only dental professional association worldwide that has established a scrutinizing protocol that verifies the skill level of a cosmetic dentist. The AACD accreditation process is humbling, yet inspiring and rewarding.
It has given me confidence in my work and the momentum to constantly further my level of experience.

Most importantly: It is always about and for my patients.

Carefully listening is the most important step towards any rejuvenated smile. Only once I truly understand what my patient desires or hopes for, can I apply what I have learned throughout my years with the AACD, my various mentors, such as Dr. Peter Dawson (founder of the Dawson Academy in St. Petersburg, Florida), and my personal experience while helping my patients.
