A smile is composed of static and dynamic elements.

Lips, facial muscles, and the masticatory neuromuscular system are the dynamic parts. Teeth, gingiva, and alveolar bones are the static parts.

In dental occlusion and function, the pillars of support refer to the fundamental components that maintain the stability, function, and health of the masticatory system. The pillars ensure proper force distribution, occlusal harmony, and long-term health.

These pillars include the vertical dimension of occlusion, anterior guidance, occlusal stability, temporomandibular joint health, neuromuscular harmony, and periodontal support.

Lips that are quite independent of the pillars of support. Unlike teeth, bones, and TMJ, lips are highly dynamic soft-tissue structures that extremely vary from person to person. They are involved in activities like speech, swallowing, and, of course, facial expression that we all care about so much.

This is underlined by the different hardwiring of structures that are supplied by the Facial Nerve and the ones that are supplied by the Trigeminal Nerve.

The primary function of the Facial Nerve is facial expression. The primary function of the Trigeminal Nerve is sensory of the face and oral cavity, and motoric for the muscles of mastication.

While the cosmetic dentist can set the “stage” for a new and rejuvenated smile, he is unable to change the dynamics of the “curtain.” In the above case, there is some asymmetry in the function of the smile muscles, as zygomaticus major, zygomaticus minor, levator l***a superioris, levator anguli oris, and risorius. They are supplied by the Facial Nerve, and it is not uncommon the one side “fires” differently than the other side. In extreme cases, a person might suffer from Bell’s palsy, which leads to a lopsided smile.

Cosmetic Dentistry is not just a smile makeover or a set of porcelain veneers to rejuvenate one's teeth and general looks.
The mouth is the most dynamic and exposed area of the human body. It analyzes anything that intends to enter the oral cavity. It communicates. It processes, triturates, and distinguishes the consistency and chemistry of a bolus that entered the mouth.

However, any new smile has to be fully integrated into the harsh environment of the guardian and processing plant of any object that wants to enter the digestive tract.
This case shows the result of decades of exposure of teeth to erosion and parafunctional attrition. Teeth deteriorated at an accelerated pace, leading to a premature aging of this patient’s smile.

Our focus on beauty often distracts us from malfunctions that lead to a deterioration functional body units, such as our teeth and jaws.
Cosmetic dentistry can only be applied after discovering functional causes that contributed to a breakdown of oral and dental structures. The rehabilitation of proper function needs to be completed before a smile is rejuvenated.
This case illustrates such functional breakdown, even though the patient just wanted to get his worn teeth rebuilt, the asymmetries corrected, and the diastemas closed. He also felt that his teeth looked too small. He had no idea about the causes of this deterioration.
It was determined that he had a dysfunction of his chewing apparatus. His temporomandibular joint were forced out of their home position by a hyperactive "GPS" within his central nervous system and hyperactive mandibular positioning muscles. After educating him about the causes of his dental breakdown, the neuromuscular dysfiunction was addressed first. Once his neuronal positioning system was calmed, his teeth were restored in harmony with the recovered home position of his temporomandibular joints.
He now is comfortable, is not aware of griniding his teeth anymore, and will be able to enjoy his new smile for many years.

Some like it white 😁!

Some patients, who have had naturally dark/yellow or stained teeth during their entire lives, just want the brightest opposite. They have made up their minds far before they meet with me.

"White yet natural" is their motto, and they look for someone who in their opinion would achieve that goal. Lucky me that they have found me. And apparently lucky for them, because they usually leave very happy with their new smiles.

Here is an example of one of the more drastic makeovers towards a brighter smile. The patient had his goal set. We talked for hours, I helped him visualize with images, and he tested with temporaries. His choice remained to have that "stellar" smile. So, he got what he was looking for and left the office a happy man...
