Important Notice Regarding Potential Data Breach of Patient and Employee Records
To our valued patients and former staff members,
It is with deep regret and concern that we inform you of a potential data breach involving the unauthorized removal and disposal of sensitive records from our dental office.
According to a witness employed by the landlord, On or around May 12, 2025, the landlord of our leased space:

took it upon himself to enter and remove all contents from our dental office without following proper legal procedures or data protection protocols. These actions were not sanctioned by our practice and occurred without prior notice. Over the last year the practice has engaged in multiple attempts to remedy the situation of the illegal lockout that occurred and recover records being withheld by the landlord.
On May 21, 2025, a significant number of documents were discovered in the dumpster behind the dental practice. These included thousands of patient charts, insurance claims, and documents containing Protected Health Information (PHI), as well as employee records, W-2s, payroll records, and other Personally Identifiable Information (PII). Additionally, our server and several workstation computers were found in the dumpster with the hard drives removed. We are currently unable to determine the full scope of data that may have been compromised or who may have accessed this information during that time.
This breach has been reported to the San Antonio Police Department, and the following case numbers have been assigned: SAPD25101020 and SAPD25101645. The State Board of Dental Examiners, Texas Department of Health and U.S. Department of Health and Human Services (HHS) have also been notified of the breach.
We are actively working with authorities and data privacy professionals to assess the extent of the breach, recover any remaining information, and take all necessary steps to protect your data moving forward. If you believe your information may have been compromised, we encourage you to monitor your credit and insurance activity and consider placing fraud alerts or credit freezes if necessary.
We sincerely apologize for this incident and the distress it may cause. Our commitment to safeguarding your personal and health information remains unwavering, and we will continue to provide updates as we learn more.

1. File a Complaint with the U.S. Department of Health and Human Services (HHS) Office for Civil Rights (OCR)

The Texas AG handles violations of consumer protection laws, including data breaches involving Personally Identifiable Information (PII).
