What does it mean when your dentist says they want to watch a tooth?

It means we can see a cavity starting, and there's still time to stop it.

On your x-ray there's a spot where the enamel has begun losing minerals and going soft. There's no hole yet. It hasn't broken through to the softer layer underneath. At that stage, the tooth can still pull those minerals back in and harden up again.

So watching means we're standing in the window where the outcome can still change. We'll talk about what's causing it, we'll go through your brushing and flossing, and in a lot of cases we can treat the spot directly to help it heal.

Then I look at that same spot on your next set of x-rays to see which way it went.

What if you could try on your new smile before you spent a cent on it?

You can. It's called a smile trial, and we can do it right at your consultation at my Salem dental office.

We make a temporary version of what your teeth could look like and slide it over your own teeth. You look in the mirror. You take photos from every angle, in our light and out in the parking lot, and you sit with it for a bit. Most people take about nine pictures before they say a word.

Then you go home and think about it for as long as you want.

You don't have to be one of our patients to come do this. If you're just curious what a change would look like on your own face, that's a perfectly good reason to book.

Sometimes I have to tell a parent we should call the orthodontist and have the braces taken off early. It's never an easy conversation.

When that conversation comes up, the situation is usually this, a kid has braces, and the teeth underneath the brackets aren't being kept clean, despite our hygienist's best efforts to coach technique, despite all the tools we send home. We can see plaque sitting on the teeth, and we can tell from the color and texture how long it's been there. The teeth underneath are starting to take real damage.

At a certain point, leaving the braces on is doing more harm than good. The braces are doing what they're supposed to do, but the hygiene around them isn't keeping up, and the call has to be made.

I know how hard this is to hear if you're the parent. You've put in the time, the money, the appointments. You've done everything right. The kid isn't ready to take care of teeth that need that level of care, and that's not a failure on your part. That's a developmental thing, and we can revisit ortho in a few years when they're ready.

I never want this conversation to feel like blame. It's a duty-of-care call I make on behalf of the kid's adult teeth, and I make it gently.
