This case was previously accessed before the referral because the patient was experiencing pain during treatment. I knew the distal had a 90 bend in it from the CBCT and a close proximity to the IAN. The mesials are actually a 2-1-2 system. I was never able to get patent in the distal. Ran a 5% GW cycle and obturated the case. Love to see all the accessory canals too.

This is why I try to use the for each case that I treat. I’m not trying to sell any units btw, just trying to do the best I can for my patients. As I said in an earlier discussion with the EndoActivator video, I’m better with the technology than without. I would have had no shot at cleaning the DB split (which I was unable to even identify on the CBCT). By using it, I was able to clean spaces I was not even aware were there. How clean can you get this system without advanced irrigation? Good luck with a swizzle stick. We saw how that doesn’t work in a curved canal. Thanks . How about the MB system? Is cleaner better? Probably. Is dirtier better? I don’t believe it is. So, if I can treat a tooth with conservative shapes, clean the system as best as I can and then fill the irregularities with a sealed restoration, what more can we do for our patients to give hem the best chance for success? This is how I would want my tooth treated. And if a better technology is available tomorrow, I’m open to new ideas.

Here is a case from yesterday. I could never achieve patency in the mesial root. It was a “wishbone” system with multi planar curves. You can see the cone fit radiograph. However, if you can obturate the system with materials that flow, sometimes you get results like this. Using a conservative approach with GentleWave seems to give me the best results.

If I’ve learned anything from using GentleWave to its greatest potential, I’ve learned to stop messing with apical anatomy and let the machine do what it was designed to do. In this case, I achieved patency with a 10 HF and debulked the canal space. I was in the tooth only for a few minutes before I was running the GW cycle.

This case was treated initially 1 year ago but the tooth “never felt right.” The CBCT tells the story of why. 1 visit retreat with GentleWave. I was only able to get patent in the buccal root with an 8 HF on a couple occasions. The final radiograph shows that we even picked up an accessory branch in that root with a system “S” obturation technique.

Minimally shaped case from today. DC Taper 17/.04 in all canals except the DB (20/.06) Shaping cost a few minutes. Minimally shaped case from today. DC Taper 17/.04 in all canals except the DB (DC 20/.06) from We ran GentleWave cycle with the traditional Molar PI. Shaping cost a few minutes. Irrigation cost 9 minutes. Accessory canals in the mesial system and in the radix, priceless. Great process centered outcome. We’ll check healing at 1 year.

This case was completed over 2 visits. I was able to locate 3 canals at the first visit and then found the DB at the second visit. I used a Molar PI once the canals were located and shaped. The CBCT demonstrated bone loss extending back to tooth #2 and the patient had swelling distal to tooth #2 after the first visit. I am hopeful that this treatment will allow the area to heal successfully and completely.

We had the new G4 unit installed at our office today. We are so grateful for the 5+ years spent with the G2. It was practice changing for our office. Looking forward to the coming years of multi sonic cleaning with our new equipment.
