Counting some blessings

Since I started working part time (around 100 days per year) about a decade ago, this is a typical daily schedule. The first patient is the wife of another patient of mine who had extensive dental implant / bone grafting treatment done by me. No better compliment than seeing the schedule with patients who were referred by family members.
Dental or medical specialist training takes a very long time, and involves very hard work. Passing the hundreds of exams necessary to get licensed is probably the easiest part of this effort. It’s the work / life balance that suffers the most i think. My family should know about this very well. My daughter Alie, MD, as a thoracic surgeon, son Michael, MD, as a pediatric neurologist, me, DDS, MS, as a periodontist, and my better half Sam, RN, MS, as a healthcare management specialist who gently manages all of us in a diplomatic and often unappreciated way ❣️

Extraction of maxillary molar, Sinus Lift and Dental Implant Placement

With this procedure we are able to shorten the treatment time by 9 to 12 months, if a patient qualifies for this kind of treatment. These two cases were both done last Friday, and they went very well.
In addition to a basic extraction kit i only used the instruments shown in the last picture (osteotome and dental implant drivers). There was no need for a dental implant motor or a complex surgical kit (which I had as a backup) since the dental implant was placed in the tooth socket instead of a machined opening.
After the extraction the bone between the three root openings (upper molars usually have three roots) was slightly modified to make room for a 3mm wide osteotome which was gently tapped with extraction forceps (a surgical mallet can also be used) to raise the floor of the sinus.
Hydroxyapatite, a powdered bone grafting material, was used both as a cushion and as a filler to raise the floor of the sinus (the lining of the sinus aka Schneiderian membrane) to a desired level. The patient’s own osteoblasts will migrate between the grafted particles, and will make new bone creating additional support for the dental implant.

Always inspired and energized by professional conferences👍
So far lectures were about basic sciences and cellular biology (👌up my alley), and attendees are mostly researchers and oral surgeons.
And venues like this (Silverado Resort in Napa, California) are a little bit of a justification for working hard (maybe too hard sometimes) for so many years. Later tonight a few top tier wineries will send their people over, and they'll put together a wine tasting event for us 🥂 Look forward to it.
Sam, Jane, Drew (periodontist in Dallas, Texas) and I did similar tastings in four (wow, just counted😁) different countries so far. So we are getting half decent at it🥂

Teeth with gum recession rarely hurt, and the look doesn't really bother the average person, so is this a problem that needs to be fixed?

Glad you asked. The before and after first two pictures show an example of gingival recession repair with gingival (gum) grafts, very common dental surgery procedures.

Left untreated these defects cause problems that go way beyond compromised esthetics.

The third picture shows teeth with root decay and attempted repair with tooth colored fillings that don't have a good long term prognosis. In the fourth picture l used a measuring instrument to check the depth of the decayed area. It was 5mm deep, so it's "game over" for that tooth. Replacing this tooth with a dental implant is both a more expensive and more extensive procedure than a gingival graft repair done at the right time.

Since Dr. Bonilla joined our team earlier this year our practice is significantly enhanced by focusing more on each doctor’s areas of expertise.
Dr Carlos E. Bonilla graduated from Boston University Dental School in 1993. He joined the US NAVY as a dental officer and served as part of First Dental Company, First Dental Battalion, Camp Pendleton CA until 1997, gaining experience in general dentistry and surgery.
While in California he started working as a civilian in private practice. Now with 25 years of experience doing all aspects of general and cosmetic dentistry, he also has training in implant placement and restoration.
Dr. Bonilla is married to Dr. Diaz and is the proud parent of two young men aged 17 and 21. He coached them in baseball for some 8 years. He enjoys sports, water sports and baseball. His passion is dentistry and serving the patient first.
Our Downtown Sacramento office is located in an area called Mansion Flats, so it was decided to rename the office to reflect its specific location.

GUM RECESSION REPAIRED WITH MINIMALLY INVASIVE SURGERY

Unstable gum recession sites can be repaired by grafting the defect with soft tissue rich in collagen fibers. The goal is to thicken the epithelium, and, fully or partly, cover the exposed root surface.
In this case the patient opted to have grafting done using her own tissue harvested from the roof of her mouth vs. using material purchased from a tissue bank. The advantage is that the outcome is more predictable and the disadvantage is a second surgery site.
However, since the connective tissue is harvested underneath the epithelium, when the site is sutured it looks like one single incision with minimal discomfort😊

EXPANSION OF NARROW JAW BONE RIDGE BEFORE IMPLANT PLACEMENT

When I looked at the schedule a few days ago this case was supposed to be the easiest (as most premolar dental implants are) out of the seven dental implants I placed that day.
However the width of the bone ridge was too narrow for typical implant placement, so a deep slot was created to expand the width of the ridge using a wedge like instrument.
Sounds easy, but if the bone doesn’t want to cooperate, and it decides to fracture, the planned treatment (implant placement) needs to be aborted.
Everything went well, and the slot opening was grafted after the dental implant was successfully placed 😊

ABOUT THE CARE PART IN HEALTHCARE
Ann has been a patient of ours for a few years now, and I knew that she was diagnosed with stage four breast cancer and that she is on chemotherapy medication. A small detail, however, was missing from her medical history, and she casually mentioned that to me a couple of days ago; the fact that she was diagnosed with inoperable stage four cancer twenty years ago😲
I don’t think that it’s a coincidence that her son is an Oncology Pharmacist with a major hospital in the area. He recommended adjustments and changes in his mom’s medications, and he was able to turn this feared diagnosis into a manageable chronic disease🙏👍
