I like explaining cavities in terms of severity to patients. On a scale of 1 to 10, the tooth 2nd to the right would be a 2 or 3 which is easy to fix and may not even need to be numbed! The very right tooth might be more like a 6, which means it'll require a larger filling, being numb and possible sensitivity after treatment.

Once you start getting to 8 or 9, the tooth is expected to hurt constantly and a root canal may be necessary.
