There are few phrases in dental billing that hurt quite like:

“Denied for timely filing.” 😭

Because unlike a lot of denials, this one can be very hard, or impossible, to fix.

Every carrier has its own filing requirements, and some give you a generous window while others… absolutely do not.

That’s why claims that are rejected, missing information, or sitting unresolved need to be followed up on BEFORE the filing deadline becomes the problem.

A claim sitting quietly in AR isn’t necessarily harmless.

Sometimes it has an expiration date. ⏰

That 90+ day insurance balance isn’t getting better with age. 😬

The older a claim gets, the more likely the follow-up gets complicated.

Maybe the carrier needs documentation.

Maybe something was never received.

Maybe the claim processed incorrectly.

Maybe it was denied weeks ago and nobody caught it.

Or maybe it really is “still processing” for the 47th consecutive day. 😂

Aging reports aren’t just a list of old balances.

They’re a roadmap showing you where money may be getting stuck.

And the sooner you figure out why, the better chance you have of actually collecting it.

Two dental insurance plans should mean twice the fun, right?

😂 No.

Welcome to Coordination of Benefits.

When a patient has more than one plan, figuring out which carrier is primary is only the beginning.

Then you have to know how the secondary plan coordinates, whether they need the primary EOB, what they’ll actually consider after the primary payment, and whether their system agrees with any of the information you were given in the first place.

Two plans can absolutely help a patient.

But they can also turn one claim into a small research project. 😅

Ahhh, the pre-authorization (or as we call them, Pre-Estimate!).

Insurance says: “Estimated benefit: $1,200.”

Everyone feels good.

Treatment is completed.

Claim is submitted.

Insurance: “Anyway… here’s $743.” 😂

A pre-auth can be incredibly helpful for planning treatment and estimating benefits, but it still isn’t a guarantee of payment.

Benefits can change. Other claims can use the annual maximum. Eligibility can change. Frequency or plan limitations can come into play.

So yes, absolutely get the pre-auth when it makes sense.

Just don’t let the word “authorized” trick anyone into hearing “guaranteed.”

Dental insurance loves a disclaimer. 😅

A zero balance feels very satisfying.

But a zero balance doesn’t automatically mean the account was handled correctly. 👀

Sometimes it means the insurance paid.

Sometimes the patient paid.

Sometimes the correct contractual adjustment was made.

And sometimes somebody adjusted the remaining balance because no one knew what else to do with it. 😬

The goal isn’t simply to make every account hit $0.

The goal is to make sure it gets to $0 for the right reason.

That little distinction matters a whole lot when you multiply it across hundreds of accounts.

A claim says **PAID.** ✅

Great. Done, right?

…not necessarily. 😅

A paid claim can still be processed incorrectly.

Maybe the wrong fee was allowed. Maybe a procedure was downgraded. Maybe something bundled that shouldn’t have. Maybe the patient portion doesn’t match the benefits you were expecting.

That’s why posting the payment is only one part of the process.

The EOB still needs to be read.

Because sometimes the difference between “insurance paid” and “insurance paid correctly” is money sitting quietly in your AR.

We verified the benefits. ✅
Insurance: “This is not a guarantee of payment.”

Dental offices everywhere: …then what exactly did we just verify? 😅

Benefit verification is incredibly important, but it’s still only a snapshot of what the plan is showing at that moment.

Once the claim actually processes, things like frequency limitations, downgrades, prior usage, annual maximums, coordination of benefits, exclusions, or missing history can still change the final payment.

So yes, verify the benefits.

Just remember: verified does not mean guaranteed.

Dental insurance really does love giving you information… and then immediately adding a disclaimer to the information it just gave you. 😂

Dental insurance math has a way of making everyone question their calculator. 😂

A plan says it “covers 50%,” so it’s easy to assume insurance will simply pay half of the office fee.

If only it were that simple.

The actual payment can depend on the carrier’s allowed amount, deductible, frequency limitations, downgrades, alternate benefits, exclusions, annual maximums… and whatever else is hiding in that particular plan.

So when the insurance payment comes back lower than expected and there’s still a patient balance, it doesn’t necessarily mean something was billed incorrectly.

It’s also why we always remind offices that an insurance estimate is exactly that: an **estimate**. The final patient portion isn’t truly known until the carrier processes the claim.

Dental insurance: where “50% covered” somehow requires significantly more math than 50%. 😂
