Dear Patient,
I am writing to inform you of some changes coming to our practice. In order to better serve our patients, we have decided to join another full-service, private dental practice at a new location.
We will be joining Dr. Vincent Zammuto and Dr. Jon Szleszinski at the Mulford Dental Group located at 4001 North Mulford Rd. The office is located on the corner of Mulford and Riverside. Mulford Dental Group is a full-service practice also offering cosmetic and orthodontic services. Mulford Dental Group has expanded evening hours for more convenience. Although I will be working out of the Mulford and Riverside location, they also have an office at Perryville and Newburg Rd (Redansa Dental) where you can be seen for your cleanings and dental work as well. The Doctors at Mulford Dental are:
Dr. Vincent Zammuto: Dr. Zammuto is a Rockford native. He Graduated from Guilford High School then Rockford College with a B.A. in Chemistry. He then went on to graduate from Marquette University School of Dentistry in Milwaukee. Dr Zammuto lives in Rockford with his wife and has practiced dentistry here since graduating from dental school.
Dr Jon Szleszinski: Dr. Jon was born and raised in Rockford, Illinois. He graduated from East High School and then went on to Rockford University and received a B.S. in Biochemistry. He graduated at the top of his class at Midwestern University College of Dental Medicine Illinois. He lives in Rockford with his wife and 2 children.
We look forward to a continued partnership with you in this new endeavor. Our goal is to continue to provide you with the same personal service you have previously enjoyed in helping you maintain good dental health in our new location.
