It is with great sadness that we announce the following:
Dear Patients,
Please accept this letter as notice that New Smile Dentistry will permanently close effective July 7th, 2023. We have enjoyed being your dentist and will miss working with you.
There are many talented dentists in the area. Please consult your insurance company's list of approved providers to select a new dentist. Your dental records are confidential and will be securely maintained in compliance with state and federal regulations. A copy of your records can be transferred to another dentist or released to you or to another person you designate, with appropriate authorization. An authorization form is enclosed for your convenience. Whether you want your records transferred to another dentist, or whether you want a copy, please fill out the authorization form and email it to: [email protected].
