Diagnosis: Pulp Necrosis, Chronic Apical Abscess
CBCT imaging revealed a periapical radiolucency at the mesial apex, along with an additional radiolucency and associated bone loss along the distal root. Clinically, a sinus tract was noted near the CEJ on the buccal and distal aspects of the crown, and periodontal probing revealed a 10 mm pocket on the distal.
Upon access, three canals were located; all were calcified and necrotic.
At the subsequent visit, the patient returned for evaluation prior to obturation. The patient reported no symptoms. Clinical examination showed complete resolution of the sinus tract and distal periodontal pocket.
Obturation was completed, and the patient was placed on a 6-month recall schedule.
