Our patient returned for her second connective tissue graft after a positive experience with her first grafting procedure with us. This time, we grafted her lower premolars utilizing the minimally invasive tunneling technique and achieved complete coverage of the exposed roots along with phenotype modification. Conversion of the gingival phenotype from thin to thick is the most important biologic outcome for any soft tissue grafting technique for preventing progression of recession. This patient always heals well because she adheres to post-operative instructions and takes medications as directed. Once we do our part to educate and execute a rigorous, clean surgery, the rest is in the hands of Mother Nature and the patient.

We had a terrific Seacoast Hygiene Study Club meeting last night with nearly 80 of our colleagues! There were many insightful questions and a great conversation. Our topic was Saving Teeth, which is our mission and theirs. Dental implants are a miraculous tool to rebuild a damaged dentition, but we are always gratified when we can help our patients not get to the point of needing them. Thank you to all who attended!

Our patient had progressive recession on her mandibular incisor, which is one of the most challenging areas to treat with soft tissue grafting due to strong muscle activity and often high frenum attachments and shallow vestibular depths. For these reasons, we often treat these sites with the conventional free gingival graft due to its robustness. In this case, however, we performed connective tissue grafting with a frenectomy and achieved complete root coverage and a seamless blend of new and existing gingiva, which often cannot be achieved with free gingival grafting. Our patient was very pleased with the ease of the procedure and the improved comfort, health and esthetics of the area post-treatment.

Our patient had an abscess on his lateral incisor implant and recession on his adjacent canine. Teeth and implants with thin mucosa are more at risk for periodontal infections than those with thick, supportive soft tissue. We performed surgical debridement of the implant threads and augmented the soft tissue around the implant and canine with a connective tissue graft. We achieved resolution of the infection on the implant, coverage of the exposed canine root and increased thickness of soft tissue, protecting the area from future issues.

This is the beauty of the tunneling technique for connective tissue grafting. Our patient had a history of aggressive toothbrushing that led to deep recession and jeopardized the health of his teeth. We educated him on proper gentle toothbrushing and treated the recession with a minimally invasive connective tissue graft, completely covering his exposed roots and augmenting the soft tissue to prevent relapse.

Our patient was referred to us for placement of an upper first molar implant in proximity to the maxillary sinus. The video on the first slide shows the cone beam CT scan, which revealed inadequate bone volume for implant placement in the ideal, restoratively-driven position. In the scan, the virtually planned implant (shown in red) can be seen extending into the sinus cavity. After surgical planning with our dental lab (slide 2), we placed a fully guided implant in the precisely planned position with an internal sinus lift. Internal sinus grafting allows for minimally invasive elevation of the sinus membrane, placement of bone graft material to increase the vertical dimension of bone and simultaneous implant placement. As expected, our patient had minimal to no pain post-operatively and the implant will be ready for its crown in just three months.

Our patient presented with recession on her upper canines. Canines are keystone teeth of the dental arch and should be carefully protected. We performed connective tissue grafting utilizing the minimally invasive tunneling technique to cover her exposed canine roots and increase the soft tissue thickness, which will prevent further recession. We now expect our patient to maintain these critical teeth for her lifetime.

Our patient had mild gum recession around his lower left bicuspid tooth, moderate recession around his lower left canine, and severe recession with formation of a fissure (Stillman’s Cleft) around his lower left lateral incisor. All three teeth lacked a healthy band of normal attached gingiva (gum tissue). The lateral incisor was at high risk for infection if food or plaque became trapped in the fissure. Using a microsurgical grafting process, we regenerated a significant amount of attached gingiva and completely closed the cleft. He told us that the procedure was painless and that he is amazed at how much stronger his gums feel.
