In March of this year, the Washington Association for Community Health and the Center for Accelerating Care Transformation (ACT Center) launched the Pathways to Better Diabetes Care Collaborative (https://na2.hubs.ly/H04nH9k0), bringing together community health centers throughout the state to implement strategies to improve diabetes outcomes.

The group held its first in-person meeting recently. NOHN was represented by Stacie Neff, Quality Director; Tricia Gormley, BSN, RN, CDCES, Clinical Nurse; and Heidi Freislich, Nursing Supervisor. Stay tuned for updates on this important work.
