Hygienists and doctors! Please don't let anyone extract your patients teeth and/or cut their gums open until you have explored Regenerative Periodontal Endoscopy as an option for your patient. Saving teeth deemed hopeless is what I love to do! This is why people travel from far away places...wish I could clone myself thousands of times!

Easy full mouth case for me (7-14mm) but I wanted to point something out. The distal of #31 had a 3rd molar communication defect measuring 14mm and an advanced furcation on the buccal you can see in the picture below. But notice you do not see this all that much in the x-rays. This is why accurate probing is so crucial for diagnosis. Also, this tooth had zero mobility yet the periodontist wanted to extract it. Hygienists and dentists, are you letting your patients undergo extractions because you feel there is no other option? Just know that there is!

Big day at Peninsula College with the ribbon cutting ceremony for the new Dental Hygiene Program.

Kudos to all the donors and the amazing individuals who worked so hard to bring this program to fruition.
This is such a huge gift for all of Clallam County! They will start seeing patients this winter! 🙏🥰

And bonus, the endoscope technology will be introduced next year 😁

Lately I've been seriously questioning WHY I keep going on this very difficult journey of PerioPeak. It has never been an easy path to do this work and stay the course. But this morning I had to remind myself of my decades old intention to bring non surgical options to patients like this routinely and how I was told I would never be allowed to do this work in the periodontal specialty practices I worked in.

I did this case in general practice as PerioPeak over 20yrs ago to uphold the gold standard of non surgical periodontal therapy and provide a place of patient autonomy (the right to choose non surgical therapy over extractions and surgery). Obviously this patient did not want to extract this molar. This defect healed nicely in only 3 months after RPE. I haven't looked back since. I have treated thousands of severe case successfully that would otherwise have had expensive surgery, extractions and implants. Many of them full mouth extractions. We don't find out what is possible by staying some comfortable, claustrophobic box.

But I am once again feeling at the end of my short rope to abandon this intention because it has been so incredibly difficult to find a place that will let me do this important work and respectfully run my business. I cannot give up, and I won't! 😁

Today is the 25th anniversary of my Mt Rainier summit and the inspiration and beginning stages of PerioPeak Innovations. Woo hoo! ✨🙏😁

I just wanted to share a little of this story because I feel it's important. I grew up in the foothills of Mt Rainier and always wanted to climb it, so when the opportunity came to take a glacier climbing course with a great group of people I jumped on it. Six months of hard core training and learning to be uncomfortable and driven made me a stronger person. I noticed that my experience and courage on the mountain had enormous impact in my clinical and professional pursuits (it permeated my entire life really). At the same time I was learning these new mountaineering skills I was teaching myself to use the dental endoscope, trying different methods, creating protocols, doing severe cases, always pushing the envelope, etc. and in time I wanted to shout from the mountain tops that all of my colleagues could do this new treatment method. If I could do it, anyone could do it! I formed PerioPeak with the purpose of teaching these new found skills and protocols to the masses. Little did I know that there would be very little to no interest at that time and that the endoscope technology itself would struggle to survive (and we are now on the 6th company about to give it a go). Since I did not have enough business for a consulting/teaching business model, I switched gears and created a business model aimed at helping patients myself. It just so happened that my patients would all be flying in from all around the globe. This wasn't my original intention but it's interesting how it turned out because I have learned so much in 25 years from my thousands of traveling patients. Doing ever more difficult cases (especially traveling patients) teaches you so much... how to be a thorough and highly efficient clinician, and discovering what is actually possible in non surgical periodontal therapy in severe cases.
I realize now that I had to go through this journey just as it happened to get to where I am now (and oh boy what a challenging journey its been - 2008 recession, winning a law suit against the DOH in 2013, and covid in 2020).
PerioPeak survived and I am still here helping patients traveling the globe. It's been my sincere honor and I greatly appreciate all the referrals and trust all these years. 🥰🙏 Integrity and results are very important to me.

My tray set up 26 years ago vs my tray set up I've been doing for the past 25 years now. Notice, no hand instruments at all. My endoscope taught me this very efficient systematic approach by treating thousands of severe cases. It's incredibly fast and easy on your body. It's also completely non invasive for your patients tissues and roots, hence faster healing and better overall results. This technique requires training. All hygienists should learn this (it's a win-win-win), there's no reason for you to have to work so hard to achieve a biocompatible root surface. I am speaking with Noel Paschke next week about all this since she just published an article in RDH and it's probably fresh on hygienists minds. If you want to come shadow me and learn this please reach out. Otherwise I am still planning to do an educational video (I have created several but they are not professional enough and I am a bit of a perfectionist, LOL).

Also note that the endoscope explorers on my tray are not the current crappy ones being sold. These are from waaaay back when the explorers were higher quality and were bendable to see all deep distal defects. You only need 2! The current training and equipment is not efficient and wears out the fiber way too fast. Of course the new endoscope will be entirely different as I have mentioned so there will only be one explorer for all surfaces. 😁

Also note that the only dental explorer on my tray is for the dentist to do his/her exam. ☺️

The pref gel and Emdogain are the icing on the cake! There is no need for other things (desiccation, Arestin, etc.

Love this text from this amazing dental hygiene student. I realized the power of clinical observation when she shadowed me recently. I told her that watching the RPE procedure and learning this comprehensive care would help with her entrance interview for dental hygiene and I feel it did. She got to witness the future of definitive dental hygiene and holistic periodontal therapy. This student is the future of the dental hygiene profession and I couldnt be more thrilled!! Found this old pic today (May of 2003 at one of my perio offices in Seattle). This was our 2nd endoscope, the DV2. Notice how we had to record and capture images (into my laptop). And that old diode laser was huge! LOL notice the old film x-rays and paper chart 😂

I had already made the complete shift to satelec piezo at this point (dumped all my hand instruments). So much more efficient than any other ultrasonic, and I have used many...☺️

Now that I am treating local patients at Irwin Dental Center I will be able to help this community more and monitor case...

Now that I am treating local patients at Irwin Dental Center I will be able to help this community more and monitor cases very closely, which I love doing. I am very happy about this! I like to demonstrate with pics just how fast my patients heal with my minimally invasive treatment method (Regenerative Periodontal Endoscopy). I treated the right side on Friday and she returned on Monday to complete the left side. This bony defect on the straight facial of #7 was abscessed and was causing her considerable discomfort. It healed and reattached in 3 days. Treatment time less than 10 minutes and a fraction of the cost of surgery with no down time and no change to diet. I fully expect the bone to regenerate here, and notice that there is no recession (tiny piezo tips are key). I love my work! 😍 I want to teach this to everyone! 😁
