* Each new patient must give the name of the family who referred them when calling to schedule
* All 5 referred patients must actually be seen and complete an appointment with one of our dentists by December 31st
* Patients who are only scheduled but have not yet been seen do not count
* The giveaway is open to Plainfield patient families only
* No purchase necessary
