Overcoming osteosarcoma defects in the head and neck often fails due to only a 37% 5-year survival rate. Patients often face multiple surgeries and are frequently left with substantial defects that can go unrepaired. Our patient endured at 7 surgeries over the last decade and relied on an obturator for the past five years. Through a free flap to close his oronasal fistula and Zygomatic implants, I was able to reconstruct his maxilla with digitally designed PMMA temps. He later had his nose reconstructed by . We have now restored his ability to eat, speak, and engage in many normal activities which we often take for granted and he’s able to lead a normal life again. Cancer sucks but we can beat it !

Zygomatic implant failures? What next ? A 66-year-old female underwent zygomatic implants using the original technique. She developed an oro-antral fistula due to insufficient bone and keratinized tissue at the ZI abutment junction, leading to chronic peri-implantitis and a referral to us. After unsuccessful attempts with a scarf flap and buccal fad pad, the implants were amputated. Zygomatic implant failures can leave patients in a difficult situation, often with limited options like retrying zygomatic implants or using a patient-specific implant. This case highlights the consequences of improper zygomatic implant placement, leading to failure. A patient-specific implant was used to remedy the situation, leveraging digital workflow to minimize the patient’s time without teeth.

The final piece of the puzzle is falling into place! The third volume of the series, focusing on complex implant solutions, is now in the printing process. Congratulations and thank you to all the amazing authors and the team at Elsevier who contributed their invaluable time and knowledge to make the latest edition of the Atlas on “Complex Implant Solutions with Grafting” a masterpiece and helping me achieve this significant milestone! This is the third and finals issue on complex dental implant cases that I am authoring and editing. Last month, I had the pleasure of sharing the stage with my dear friend .pedro.rodrigues at the full arch symposium in Dubai organized so well with an amazing lineup by another great friend Hardik Parekh. I wanted to share part of his brilliant lecture about the palatal and transsinus approaches and the biological width.

Based on of the reviewed literature, the concept, formation, remodeling, structure, dimension, and functional significances of periimplant biological width are formed within weeks of implant placement. The formation of biological width around implant is a complex process after several weeks of healing. The biological width around implants is a 3-4mm distance from the top of the peri-implant mucosa to the first bone-to-implant contact or the stabilized top of the adjacent bone, consisting of sulcular epithelium, junctional epithelium and fibrous connective tissue between the epithelium and the first bone-to-implant contact or the stabilized top of the adjacent bone. The biological width forms a biological barrier against the bacteria, influences the remodeling of soft and hard tissue around implant and has implications for clinical aspects of dental implantation. But I have found is it the biological width is often overlooked by clinicians and this can have devastating consequences as long-term.
