❣️All on X with 1.5 years follow up for a patient who has been avoiding the dentist for years after childhood trauma from dental work.
♦️7 upper and 6 lower implants were placed.
♦️Fully guided implants placements and restoration.
♦️Pterygoids implants were placed guided following Salama/Holtzclaw technique
♦️Following the digital wax up, Bite Turbos were used to open the bite 4mm to minimize bone removal.

Nothing is more rewarding than a happy grateful patient. This case really shows how much implant dentistry can change..not just patients’ smiles but their lives❗️

Happy full arch Friday❣️
♦️6 upper and 6 lower implants were placed.
Fully guided implants placements and restoration using Chrome system.
Multiple planning sessions to make sure transition line won’t show on maximum smile. Three different labs involved.
Nothing is more rewarding than a happy grateful patient. This case really shows how much implant dentistry can change... not just patients’ smiles but their lives ❤️

♦️Severely resorbed ridge from an old extraction site.
🚩A collagen membrane and periosteal sutures were used to stabilize the bone graft and membrane. Another option for membrane stabilisation would be using tacs/screws ‼️

😎 Nothing beats Free gingival graft around an exposed implant plus a new good crown.
There’s a myth saying Tuberosity grafts lead to ugly results and can’t be used in esthetic areas.
Actually, using a tuberosity graft with the correct dimensions gets you the best long lasting results..
Autogenous still is The Gold Standard!

🔋The power of soft tissue grafting prior to ridge augmentation
💣 Most common complication of ridge augmentation is exposure of membrane/bone blocks
✨ Improving soft tissue biotype decreases dramatically these complications⚒️.
✨Soft tissue augmentation also improves handling of tissues during surgery, increases blood supply and improves osteogenesis.
Here’s a Vestibuloplasty and T-FGG with 6 weeks follow up
1️⃣ Split thickness flap raised
2️⃣ Complete removal of muscle attachments
3️⃣ Apical suturing of the flap using 6.0 Nylon
4️⃣Double tuberosity FGGs sutured using single interrupted and sling mattress 6.0 Nylon sutures
5️⃣6 weeks follow up

All on 6: 4 maxilla and 2 pterygoid (16mm) implants were placed 💥
While posterior maxilla exhibits the lowest bone density, which many times delay immediate loading, Pterygoid implants add the benefit of the high initial stability for immediate loading (D1 bone) as well as extending fixed prosthesis to second molar occlusion.🥂

💥Apicaly positioned flap with double tuberosity grafts to restore keratinized tissue thickness and width 360 degrees around 4 implants.

🍴Note the manipulation of the tuberosity grafts by filleting them followed by the perforations done for perfect adaptation around the implants.

Here’s a combination case of esthetic crown lengthening along with guided tissue regeration (bone grafting) for a vertical bony defect.
⭐️Here are the steps:

1️⃣Gingivectomy.
2️⃣ SRP.
3️⃣ Bone removal (osteoplasty and ostectomy.
4️⃣ Bone grafting using allograft.
5️⃣ 6.0 Polypropylene Sutures
6️⃣ Follow up 3 weeks
7️⃣ Continue with ortho treatment after 3 months of healing.
8️⃣ Restorative evaluation for possible veneers/crowns to mask black triangle.

Pre-ortho gingival graft for a young kid. When it comes to mandibular recessions with shallow vestibule and frenum attachment, nothing beats a lateral pedicle flap with a tuberosity connective tissue graft.
⭐️ Easy
⭐️Predictable
⭐️More esthetic than free gingival grafts
⭐️Much less painful than palatal grafts

Whether to graft before or after orthodontic is always a debtable topic.

Here are some hints on when and why to graft before ortho
1️⃣ When the frenum attachment is high ➡️ graft pre ortho
2️⃣ When the gingival tissues are very thin (you can see through them) ➡️ graft pre ortho
3️⃣ When there’s a mucogingival defect (absence of attached tissue) ➡️graft pre ortho
4️⃣ When the gingival recessions are 3 mm or more ➡️ graft pre- ortho
5️⃣ When you 2 mm or less of gingival recession with good oral hygiene, 2 mm or more of attached tissue along with absence of frenum pull ➡️ re-evaluate for possible grafting after ortho treatment is done.

🌟Remember that leaving the gingival recessions to progress during ortho treatment not only leads to increase in gingival recessions but also in buccal bone which is gone for good!
