From Digital Planning to Clinical Ex*****on 🦷

Thoughtful tissue management begins before the first incision. This case highlights digitally guided connective tissue graft harvesting as part of an immediate implant workflow, with the goal of enhancing soft tissue contour and supporting peri-implant stability.

Why connective tissue grafting?

▪️ Increases soft tissue thickness and reinforces the peri-implant phenotype. ▪️ Helps maintain facial contour and soft tissue volume. ▪️ Can reduce midfacial mucosal recession, supporting a stable gingival margin.

A customized palatal guide helps transfer the planned donor location and graft dimensions into surgery. Combined with assessment of tissue thickness and individual anatomy, it supports deliberate incision placement and controlled harvesting.

Precision begins before the extraction.

This case involving tooth #13 was guided by detailed digital planning from the very beginning, with the future restoration and surrounding tissues at the center of every decision.

An atraumatic surgical extraction was performed with careful preservation of the socket walls and surrounding soft tissues, followed by guided placement of a BioHorizons conical connection implant, bone grafting, connective tissue grafting, and placement of a BioHorizons SmartShape Healer.

Every step was planned with a clear purpose: preserve the existing anatomy, support tissue volume, and establish a foundation for the definitive restoration.

A natural emergence profile begins with the plan.

Implant platform depth and restorative contour must be considered together, with the final restoration in mind from the very beginning.

Guided Apical Surgery | Fractured Instrument Retrieval — Tooth #9

A patient presented to our office with discomfort in tooth #9 approximately one year after undergoing root canal treatment in Brazil.

Radiographic and CBCT evaluation revealed a fractured rotary endodontic instrument (12 mm long) in the apical region, with part of the fragment embedded in the cortical bone of the nasal floor.

Using intraoral scan data and diagnostic imaging, we planned a guided apical surgical approach in collaboration with Tino Dental Lab ®️. A custom surgical guide was designed and fabricated to guide a 6 mm trephine for targeted access to the affected area and retrieval of the fractured instrument.

Following instrument removal, the surgical site was thoroughly curetted and disinfected. Bone graft material was placed, covered with a collagen membrane, and the site was closed with PTFE sutures.
