During this visit, we'll review your diagnosis, discuss treatment options, and answer any questions you may have. In most cases, treatment can be performed the same day, but more complex treatment plans may require a second appointment.

Referral slip & any applicable X-rays
List of current medications
Completed medical/dental insurance forms, cards (if applicable) and a valid photo ID.
