There’s a lot happening while you’re resting.
Sedation days look a little different in our office.
An anesthesiology team joins us, working alongside our team.
With everyone working together, we’re often able to complete treatment that might otherwise take several appointments in one visit.
There’s a rhythm to days like this. The room stays calm, we work together and the dentistry keeps moving.
For the patient, the appointment is quiet and comfortable.
And for those of us caring for them? We’re paying attention to everything else.

How old is too old for a filling?
There’s actually no magic number.
A filling doesn’t necessarily need to be replaced simply because it’s been there for 10, 15, or even more years. If it’s still intact, the tooth around it is healthy, and there are no signs of problems, there may be no reason to replace it.
We’re looking for things like wear or breakdown around the edges, cracks in the filling or surrounding tooth, decay developing around or underneath it, or changes that suggest the restoration may no longer be protecting the tooth the way it should.
That’s also one reason regular exams and X-rays can be useful with older dental work. They allow us to compare what we’re seeing today with what we’ve seen in the past.
Sometimes an older filling really does need to be replaced.
Other times?
It’s old. It’s doing its job. We leave it alone.
Good dentistry isn’t about replacing dental work because it’s been there a long time. It’s about knowing when something actually needs our attention.

There are some patients we’re just especially happy to see on the schedule.
She’s one of them.
She’s been coming to our office since 2022, so over the years we’ve gotten to know her, and this smile, pretty well.
We see her regularly for her hygiene visits, and along the way we’ve done fillings, crowns, whitening, and, more recently, veneers on her six front teeth.
But the best part of seeing a patient regularly over the years is getting to know the person who keeps coming back.
And that’s how certain names on the schedule become ones we’re especially happy to see.

Last week, we shared this patient’s story and his decision to make a significant change to his smile.
Now for the fun part—for us, anyway. 😏
His new smile involved 16 teeth, but this wasn’t simply a matter of placing 16 veneers.
When you look more closely at the before photos, you can see why. Different teeth presented different challenges, which meant they didn’t all need the same solution.
His treatment included a combination of veneers, crowns, and bridges, all designed together so that the different restorations would look and function as one smile.
That’s one of the things that makes a full-mouth makeover interesting. The finished smile may look seamless, but the dentistry behind it often isn’t one-size-fits-all.

“Why can't you just put a filling in it?”
Fair question!
If you've had fillings before, it can be confusing when your dentist tells you that another tooth needs a crown instead.
The difference often comes down to how much strong, healthy tooth is still there.
If a relatively small portion of a tooth needs to be repaired and there's plenty of healthy tooth surrounding it, a filling may be exactly what it needs.
But imagine a tooth that already has a very large filling, has lost part of its structure, or has developed cracks. At some point, putting an even larger filling into the middle doesn't necessarily make the remaining tooth stronger.
That's where a crown can help.
A crown covers more of the tooth and helps protect the weakened structure when you bite and chew.
So when we're deciding between a filling and a crown, we're not simply asking, “How big is the cavity?”
We're also asking:
How much healthy tooth is left, and what will give this tooth the best chance of continuing to function well?
Sometimes that's a filling.
Sometimes it's a crown.
The right answer depends on the tooth.
