CAVITIES / DECAY = Bacteria + Food + Time
Keyes Triad 1960. Still true.
Bacteria (S. mutans) + Fermentable carbs (sugar, starch) + Time (frequency) = Acid = Decay.
No bacteria = no decay ever.
Fix: Reduce bacteria load (xylitol, hygiene), starve bacteria (less frequent sugar), increase defense (saliva, pH, hydroxyapatite).

2. EROSION = Food/Acid + Time (NO bacteria)*
Acid directly dissolves mineral.
Examples:
- Dietary: lemon water, apple cider vinegar, soda, sparkling water all day, sports drinks, wine
- Medical: GERD / reflux, bulimia, morning sickness
This is CHEMICAL wear. Not bacterial.
Fix: Neutralize acid, don’t brush for 30 min after acid, saliva pH, treat reflux.

3. WEAR / ATTRITION / ABRASION = Time alone
Physical loss.
- Attrition: tooth-to-tooth grinding/clenching
- Abrasion: aggressive brushing, abrasive paste, grit
- Abfraction: flex from bite stress
This is MECHANICAL wear.
Fix: Night guard, bite correction, soft brush, non-abrasive paste.

So when someone says “I have soft enamel, my teeth just wear away”

Your mouth has 700+ species of bacteria.
If your gums bleed, you have gum disease bacteria in your saliva.
If you’ve ever had a cavity, you have cavity bacteria.

Baby is born with ZERO.

When you lick their pacifier you give them:
- floor germs
- YOUR cavity bacteria
- YOUR gum infection

Tooth cavities are a bacterial issue. Not a non-fluoridated water problem.

Let’s be logical.

Scurvy is vitamin C deficiency — give vitamin C, it goes away.
Goiter is iodine deficiency — give iodine, it goes away.

If cavities were fluoride deficiency — give fluoride, they would go away.

Do fluoridated cities have zero cavities? No. Highest cavity rates in the US are in fluoridated cities.

Why? Because the cause is still there:

1. S. mutans + Lactobacillus (bacteria)
2. Fermentable carbs / sugar (food)
3. Acid production (byproduct)
4. Acid dissolves tooth (cavity)

Fluoride does ONE thing: makes hydroxyapatite into fluorapatite — more acid resistant. It hardens the tooth. It’s topical armor.

A small filling = $250-400
You skip it, wait 2 years...

That same cavity now needs:
Root canal + crown = $2000-3000
You skip again...
Extraction + implant + bone graft = $4500-7000
You skip again...
Now adjacent teeth shift, bite collapses, TMJ pain, bone loss — full mouth rehab = $25,000-50,000

Same problem. Same tooth. Different timing. Different price.

It was never expensive. You just made it expensive by waiting.

And it’s not just money:

- Every day that cavity is open, bacteria enters your bloodstream
- Every year you have gum disease, you lose bone you never get back

You choose.

The most expensive treatment plan is the one you do 5 years too late.
