We regret to inform you that effective July 6th, 2018, we are temporarily closing our North Ogden practice. For the time being, we will no longer have staff available at this location to attend to you after that date.
At this point in time, we are referring all current patients to our BOUNTIFUL location for continuing care. In addition to assisting in completing any necessary treatment, the Bountiful office can assist you in acquiring and/or transferring your patient records in order to provide any necessary continuing assistance.
