These wedge-shaped grooves near the gumline are called abfraction lesions—a type of non-carious cervical lesion (NCCL). They're caused by tooth flexure from grinding (bruxism) or a misaligned bite, which creates microfractures in enamel and dentin. Over time, abrasive brushing or acidic exposure can worsen the damage. Even if painless, they weaken tooth structure, trap plaque, and may lead to sensitivity or fractures. Treatment involves correcting bite forces (e.g., night guard), gentle brushing, and possibly restoring the area with composite or glass ionomer. Early management preserves tooth health, prevents complications, and avoids more invasive treatments later.

𝗖𝗵𝗿𝗼𝗺𝗼𝗴𝗲𝗻𝗶𝗰 (𝗣𝗲𝗱𝗶𝗮𝘁𝗿𝗶𝗰) 𝗦𝘁𝗮𝗶𝗻𝗶𝗻𝗴 𝗢𝗳 𝗧𝗲𝗲𝘁𝗵 "

🟦 It is a form of extrinsic black staining that often appears in pediatric patients, affecting the buccal and palatal surfaces of primary and permanent teeth.

🟥 It presents as incomplete line of dark dots at the cervical third of the tooth, typically following the gingival margin, not extending to the proximal areas.

⬛ The cause is not clearly understood, but it is believed that these stains are due to reaction between iron (in saliva or gingival fluid) and hydrogen sulphide produced by chromogenic bacteria (the most prevalent types of which are; 𝑨𝒄𝒕𝒊𝒏𝒐𝒎𝒚𝒄𝒆𝒔 𝒂𝒏𝒅 𝑷𝒓𝒆𝒗𝒐𝒕𝒆𝒍𝒍𝒂 𝑴𝒆𝒍𝒂𝒏𝒊𝒏𝒐𝒈𝒆𝒏𝒊𝒄) , resulting in bacterial plaque containing insoluble ferric sulphide deposited on teeth surfaces.

Interestingly, prevalentce of caries is lower in these individuals.

🟩 Patients should be advised not to vigorously brush their teeth, as that will not ensure complete removal of stains, besides, it will cause teeth abrasion.

Instead, these stains are best removed by polishing (with pumice paste and rubber cup) followed by applying oral hygiene measures routinely.

Ultrasonic scalers should be avoided to prevent undesired enamel loss.
