This patient came in with fractured, stained fillings on her front teeth and wanted the most conservative option that would actually fix it. We landed on whitening plus six veneers, canine to canine.

The case ran through digital planning first, then same-day provisionals so she could live with the design before anything was final. Veneers are permanent, and the chance to wear the shape and length for a few weeks and say “shorter here” or “more translucent there” is the difference between a result the patient likes and one she loves.
Once she approved the provisional design, the final porcelain veneers were placed.
Good conservative dentistry is mostly planning. The less you remove, the more the design has to be right before you start.

Periodontal disease is one of the most well-documented links between oral and systemic health. The research connects it to cardiovascular disease, type 2 diabetes, and cognitive decline, with chronic inflammation as the shared mechanism.

For patients already paying attention to sleep, nutrition, and movement, the mouth is often the missing variable.

This patient was missing both lateral incisors, the teeth on either side of the central incisors. Two of the more visible spots in a smile, and two of the more technically demanding to restore.

After evaluation, I removed the remaining damaged tooth structure and placed implants to replace the missing roots. Cases like this sometimes require bone grafting to support ideal implant positioning, which matters because the long-term success of an implant depends more on what’s happening under the gum than above it. Once the implants integrated, I restored them with custom crowns matched to the adjacent teeth.

Implants are one of the most predictable procedures in restorative dentistry when they’re planned properly.

I’ve been seeing a lot of reactions to my recent video, so I wanted to clear some things up.

Cosmetic dentistry is not currently a recognized specialty in dentistry. That’s simply a fact, not a criticism of the many talented clinicians doing cosmetic work every day.

In my experience, the quality of dentistry comes down to the individual doctor, their training, their philosophy, and their attention to detail, not just the title attached to their name. I’ve seen exceptional cosmetic work from general dentists, and I recently attended ’s course in Miami, who is also a general dentist. My point was never that general dentists can’t excel in cosmetic dentistry, only that cosmetic dentistry is not currently a formally recognized specialty.

Prosthodontics is the ADA-recognized specialty that covers veneers, crowns, implants, and full-mouth restoration, requiring three years of residency after dental school, plus written and oral boards.

Cosmetic dentistry, by comparison, is not a recognized specialty. Any licensed dentist can use the title, and many do excellent work, but the training behind it varies widely.

The distinction matters most when a case gets complicated, and after thirty years in practice, I can tell you most of them eventually do.

Contrary to popular belief, brushing right after acidic drinks can do more harm than good.

Why?

Acid softens your enamel. Brushing immediately can wear that softened layer away.

Instead:
• Wait at least 30 minutes before brushing
• Or rinse with water to help neutralize pH

Give your saliva time to remineralize and protect the surface.

Your jaw activity during sleep isn’t random… it’s often tied to your airway.

When you sleep, airway muscles relax. If airflow becomes slightly restricted, your brain may trigger subtle jaw clenching or grinding to help reposition the jaw and keep the airway open.

This is a built-in survival response. Oxygen comes first, so the body will activate muscles like the jaw and tongue to protect breathing without waking you up.

Over time, this can show up as bruxism and is often linked with airway issues like Sleep Apnea.

This patient presented with concerns regarding the overall appearance of her smile and expressed interest in achieving a more natural, brighter aesthetic result.

A full restorative and cosmetic approach was completed, including the placement of 19 custom crowns to address structural wear, existing fractures, and areas of discoloration resistant to whitening treatment.

The before and after result is a balanced, natural-looking smile that restores both function and aesthetics.
