This patient presented with gum recession on teeth #8 and #9 that was already becoming visible. The roots were exposed, the gumline was uneven, and the defect was only going to worsen if we delayed treatment.
We treated this case with the Pinhole technique. There was no palatal graft, no scalpel, and no sutures. A small access point allowed the tissue to be repositioned, after which we let healing and time do the rest.
This is the result at 18 months. Root coverage has held, the contours have remained stable, and the smile looks like it belongs to the patient again. 🤜🤛

Every case forces the same quiet question:
Is this the right technique for this patient?
On the upper left, pinhole.
On the upper right, traditional tissue grafting.
Same patient. Same goal. Two different approaches.
Not because one is always better, but because anatomy, soft tissue quality, root prominence, and what the patient can realistically tolerate all matter.

We weigh the trade offs every time: predictability, morbidity, recovery, long term stability. And still, we’re making a judgment call with imperfect information.
That’s the part of surgery that doesn’t show up in the final photos.

The internal debate. The second-guessing. The responsibility of knowing that no technique is perfect and that we’re only human and trying to leave the patient better than we found them.
In this case, both sites healed well. The patient is comfortable and the tissue is stable. That outcome is what matters most.
As periodontists we share the same struggles restorative dentists face every day. We’re all in the same boat, doing our best with the tools we have, for the patients who trust us. 🤜🤛

Dirty secrets of implant dentistry that don’t get talked about enough.
Peri-implant disease is real. It’s common. And yes, implants can fail.
When they do, they often leave a large bone deficiency that makes the next treatment much harder.
This isn’t meant to scare anyone. Done right, implants still change lives. But pretending the risks don’t exist helps no one.
The more honestly we talk about these problems, the better we can prevent them and protect our patients.

Some patients walk in exhausted.
They’re tired of temporary fixes that keep failing.
They just want something that finally lasts.
Then we open the flap and find narrow bone, limited tissue, and almost no room for error.
In cases like this, technique alone isn’t enough.
It takes real respect for the anatomy, and the willingness to design the surgery around it.
At Brooklyn Periodontics, we don’t force a standard approach onto a complex situation.
We study it. We respect it. We protect it.
Because these patients didn’t come looking for another short-term fix.
They came for something that lasts.

Fully guided implant surgery has completely changed how we approach these anterior cases.
In this case we planned four implants for sites 7–10 in 3Shape Implant Studio, printed a surgical guide, and placed every implant exactly where the restoration needed it to be.
The guide gave us ideal angulation and depth control, which is especially critical in the esthetic zone where even a few degrees off can compromise the final result.
We were also able to simultaneously augment the ridge with particulate bone, knowing the implants were already locked into the correct positions.
For the restoring dentist this means screw-retained options, predictable emergence profiles, and far fewer surprises at the impression or delivery appointment. đź’Ş
