The “Hot Tooth” phenomenon 🔥🦷 ( It has nothing to do with temperature ) We, dentists, use the term “hot tooth” to describe a tooth with irreversible pulpitis, where the nerve is severely inflamed. These teeth are much more difficult to anesthetize because inflammation increases nerve excitability and lowers the threshold for pain. They often require supplemental anesthesia, even when the initial injection was given correctly.

Here are a few more scientific reasons why some patients may need additional anesthesia:

1. Inflammation changes tissue pH
This is one of the most common reasons. When a tooth is infected or inflamed, the surrounding tissue becomes more acidic (lower pH). Local anesthetics are less effective in an acidic environment, making it more difficult for the medication to reach and block the nerve. An additional injection is often needed to achieve profound numbness.

2. Bone density matters
Upper teeth are generally easier to numb than lower molars because the bone in the lower jaw is much denser. Dense bone limits how easily the anesthetic diffuses toward the nerve, which is why mandibular molars often require different injection techniques.

3. Everyone’s nerve anatomy is unique
Not everyone’s nerves follow the exact same path. Some people have accessory innervation, meaning a tooth receives sensation from more than one nerve. If only one nerve is anesthetized, another nerve may still carry pain signals. In these cases, additional injections are often necessary to fully numb the tooth.

4. Individual biology
Every body responds differently to local anesthesia. Blood flow, tissue characteristics, metabolism, and normal biological variation can all influence how quickly anesthesia begins to work and how long it lasts.

5. Anesthesia blocks pain, but not pressure. Even when anesthesia is working perfectly, you may still feel pressure, vibration, or movement during treatment. That’s completely normal. Local anesthesia blocks pain signals, but it does not eliminate the sensation of pressure or touch.

✅ DO: Arrive 10–15 minutes early

❌ DON’T: Arrive late

If you arrive late, your appointment time may be shortened because other patients are scheduled after you. Not because we want to rush you, but to stay on time for everyone. Dentistry requires time and focus, so rushing can affect quality. If you’re more than 15 minutes late, it’s usually best to reschedule so you get the full time you deserve.

✅ DO: Write down your questions and ask them during your visit

❌ DON’T: Leave with no questions, then call later with many follow-ups

It’s always okay to call later if something important comes up.

But it’s best to write down your questions and ask them during your visit, while your dentist can review your X-rays and explain everything clearly.

Callbacks may take time since doctors are with patients all day, and it’s harder to answer questions days later without seeing you.

Planning ahead makes your visit much more productive.

✅ DO: Make sure your insurance is active

❌ DON’T: Assume the office will automatically know everything

We verify benefits as a courtesy, but eligibility is ultimately the patient’s responsibility.
If your insurance is through your job, make sure you are currently employed and active on the plan. Sometimes coverage terminates and the denial happens later, which can lead to unexpected bills.

✅ DO: Listen to your dentist’s recommendations

❌ DON’T: Argue based on TikTok, Google, or ChatGPT

Online information can be helpful, but it cannot replace an exam, X-rays, and a clinical diagnosis. The same symptoms can have different causes, and the right treatment depends on what your dentist sees in your specific case.

✅ DO: Silence your phone during treatment

❌ DON’T: Take calls while your dentist is working

It’s distracting for you and the doctor, and it can slow down the appointment. Keeping your phone silent helps your treatment go smoother and faster.

✅ DO: Be honest and share important information

Tell your dental team about anxiety, pain, medications, allergies, and past experiences. The more we know, the better we can personalize your care and keep you comfortable.

This is often related to cold weather, indoor heating, and reduced saliva, not sudden dental disease.

Your mouth relies on moisture to protect itself.
When that balance changes, gums can become more sensitive and inflamed.

If something feels off this season, don’t ignore it.
Your gums are often giving early signals.

1. Master the basics (most cavities start here): Brush twice a day for a full 2 minutes using a fluoride toothpaste. Angle the brush at 45° toward the gumline and don’t rush. Night brushing is non-negotiable.

2. Floss like it matters (because it does):Cavities often form between teeth, where toothbrushes can’t reach.Daily flossing removes plaque before it hardens into tartar.

3. Watch the sugar frequency, not just the amount: It’s not only how much sugar you eat, but how often. Constant snacking feeds cavity-causing bacteria all day long. Tip: keep sweets with meals, not as standalone snacks.

4. Drink more water (especially after meals): Water helps rinse away food particles and acids. Fluoridated water also strengthens enamel naturally.

5. Don’t skip your dental checkups: Professional cleanings remove hardened plaque you can’t remove at home.Small cavities caught early = simple, conservative treatment.

6. Protect enamel daily: If you grind your teeth, use a night guard. If you’re cavity-prone, ask your dentist about fluoride treatments or prescription toothpaste.

A cavity-free year doesn’t start with perfection.
It starts with simple habits done every day.
