A J-shaped radiolucency does not always mean a crack.
This tooth 19 had a previous root canal, done by another endodontist, with gutta percha extended past the mesial apex, a large J-shaped lesion on the mesial root, and a periapical lesion on the distal. Classic crack-suspicious pattern, but there were no deep probings and the tooth was intact.
We chose to save it with an apicoectomy on both the mesial and distal roots, sealing the root ends surgically rather than extracting.
At just six months the bone had filled back in remarkably around both roots. The apico was successful and the tooth is healing.
