Dear Patients,

We hope this letter finds our patients and their families in good health and good spirits. We have all been through a lot as a community and we are ready to see each and every one of you again soon. With the reopening of our office on May 18th we want to reassure you all that we will be taking extra precautions that have been recommended by the ADA, CDC and OSHA.

- When calling to confirm patients we will be asking you a series of questions for screening as well as again when you arrive.
- Upon arrival we ask that you call our office and let us know when you are here and we will then direct you on what to do next.
- We ask that if you are bringing a guest, that they stay in the car.
- We will have hand sanitizer available to our patients
- As employees, we will have all the correct PPE to protect ourselves as well as our patients.
- We will check your temperature upon arrival as well as all employees twice a day.
- Our waiting room will no longer have magazines or toys, due to it being hard to disinfect throughout the day.
- All appointments will be scheduled to allow for social distancing between patients.

We ask that you are patient with us as this is a new adjustment for everyone. If you have any questions or concerns please call the office and we will be able to help you as best as we can.
