A patient presented with a missing maxillary lateral incisor that had failed to erupt and a neighboring canine that had drifted and erupted in an unfavorable position. Clinical and radiographic evaluation revealed an impacted lateral positioned high within the alveolar bone. Through a coordinated interdisciplinary approach involving orthodontic treatment and periodontal surgery, the malpositioned canine was gradually orthodontically repositioned into proper alignment. A surgical exposure of the impacted lateral was then performed, allowing orthodontic traction to guide the tooth into the dental arch. Over time, the impacted tooth was successfully erupted and positioned into its ideal functional and esthetic location, resulting in proper alignment, improved occlusion, and a healthy periodontal outcome. Both lateral incisors are narrow (peg) and future veneers are planned to correct this.
