How much does it cost?
Adult Program: $480/year
Age 15 and over
Includes: 2 Exams per year, 2 dental cleanings per year, All Diagnostic X-rays (including panoramic film…$150 value)
Children’s Program: $390/year
Ages 3-14
Includes: 2 exams per year, 2 cleanings per year, 2 fluoride treatments per year, all diagnostic X-rays (including panoramic film….$150 value)
Additional Benefits included:
—40% discount on all other preventive services, such as Emergency Exams, sealants, Periodontal Maintenance, Occlusal guards (bite guards), fluoride treatment for adults (age 15 and over)
—30% discount on all Minor procedures, such as fillings, scaling and root planing (deep cleaning), extractions
—20% discount on major procedures, such as crowns, veneers, root canals, bridges, dentures
—10% discount on Suresmile orthodontic treatment, Oral B toothbrushes and accessories, Oxyfresh products, and tooth whitening
The “Fine Print”
1) Annual Enrollment Fee for each family/individual is non-refundable, even if the plan benefits are NOT fully utilized during the participation period.
2) Payment must be made in full during open enrollment period in the December prior to the enrollment period.
3) Enrollment date begins on the first day of the year and provides coverage for 1 year to the date. Enrollment is available on a yearly basis only but can be renewed year after year.
4) Any costs associated with dental treatment (minus the discount) is expected to be paid in full at the time of service.
5) The exams, bitewing X-rays, cleanings, and fluoride treatment must occur within the year of enrollment and CANNOT be carried over to the next year.
6) It is the patient’s/parent’s responsibility to make and keep appointments for his/her family members.
7) This Wellness Program is NOT transferable to another party or uncovered family member.

9) All fees shall be based on Christmastown Dentistry’s UCR (usual, customary and reasonable) fees, and are subject to change.
10) The plan CANNOT be used in Worker’s Compensation Treatment Plans or treatment in general.
11) This Wellness Plan applies ONLY to treatment recommended and completed at Christmastown Dentistry and does not apply to any specialist to whom we refer.
12) If treatment is begun but not finished during the Christmastown Dentistry enrollment period, the remaining treatment will be completed with no discount applied.
Open Enrollment begins Monday December 1, 2025 and will end Tuesday December 30, 2025. You must be present in our office to sign up and pay in full. If you need a virtual option, please contact our office for more information. Our office hours are Monday - Wednesday 7:30AM - 4:30PM, Thursday 7:00AM - 3:00PM, and we are closed daily for lunch from 12:30PM - 1:30PM.
