🦷 Case Spotlight: #19 with Radix Entomolaris and S-Curved Anatomy
Patient presented with spontaneous pain on tooth #19, restored with a full coverage crown placed several years ago. Pre-op imaging revealed a radix entomolaris with severe S-shaped curvature and heavy calcification.
Access was completed conservatively through the crown. Under magnification, all canals were carefully negotiated to working length, including the radix with its double curve.
Shaping was performed with an emphasis on canal preservation and patency. Final obturation shows a dense fill—and a sealer puff from a distal accessory canal, confirming thorough debridement and apical seal.
🧠 Takeaway:
When managing curved or accessory anatomy, a centered glide path and flexible files are essential. Sealer extrusion from an accessory canal is rare—but it’s a sign of well-executed cleaning, shaping, and hydraulic obturation.
