Most patients walking in for a full-arch consultation are not new to spending money on their teeth. They have been spending it for years. A crown here, a root canal there, a bridge that lasted six years instead of fifteen, an extraction, then a partial, then another crown next to the partial. Nobody adds up the total: the patchwork, plus the time off work, plus the foods that quietly disappeared from the table.

Full-arch implants are not a small number, but neither is another decade of the same cycle. You learn pretty fast in this work that almost every adult who's been away from the dentist for a while walks in carrying something. Not always pain, not always a specific problem, but this quiet weight of feeling like they're about to be in trouble for waiting too long.

A lot of them apologize before we've even said hello, and most of them have been rehearsing the apology in their head on the way over.

The thing I want people to understand is that we are genuinely not keeping track of how long it's been since your last cleaning. We're not building a case against you.

The longer I've done this, the more I've realized that shame is the thing that actually keeps people away, way more than money or time or anything practical, and adding to it is the last thing that's going to get someone back in the chair.

If you've been gone a while, you don't owe us an explanation when you come in. You just have to walk through the door, and we'll go from there.

The patients I see most often are not the people who have let their whole life go. They are the people who optimized everything else first: the running, the food, the sleep, the bloodwork. Teeth are the one part of the body they have been working around instead of working on, usually because the last serious dental conversation ended with a number that scared them off.

Here is the thing. Teeth do not get better on their own, and waiting is rarely the free option people think it is. The patient who handles this at sixty has options. The patient who waits until seventy is often dealing with bone loss the earlier version of themselves would not have had to face. Share this with someone you love who takes care of everything except their teeth.

One of the things you learn pretty quickly in this job is that almost nobody who comes in for major treatment is actually there for the reason they tell you on the phone.

They'll say it's a cracked tooth or a bridge that needs replacing, and that's true, but it's almost never the whole story. The real reason is usually something they've been carrying around for years, like the fact that they stopped laughing in photos, or that they can only chew on one side, or that they've gotten so used to hiding their smile they don't even realize they're doing it anymore.

By the time someone finally sits in our chair, that adaptation has usually become a full second job they didn't sign up for, and the dentistry is only one piece of what we're actually fixing.

The other piece is helping them feel like a person again, instead of someone managing a thing.

That part doesn't show up on a treatment plan, but it's most of the work.

People ask me sometimes why I picked dentistry, and the honest answer is that it had almost nothing to do with teeth.

I watched my sister go into surgery, and the relationships she built with her patients were intense but really short. She'd meet someone, take care of them in this huge way, and then most of the time that was it.

I knew I wanted something different from that.

What I wanted was the kind of work where you actually get to know people over years, where you see the same families come in, where someone trusts you enough to bring their kids and eventually their parents, and where the relationship is the whole point and not just the thing that gets you to the procedure.

That's still what I love about this job, honestly.

The dentistry is the part I trained for, but the part that's kept me here for as long as it has is the people.

Watching someone go from white-knuckling their first cleaning to actually being comfortable in our chair years later, that's the part I don't take for granted.

Patients walk in here all the time with a folder, sometimes a binder, full of plans that do not agree. The surgeon said one thing, the restorative dentist said another, and the periodontist had a third opinion. By the time they sit down across from me, they are exhausted, because they have been bouncing between four offices trying to assemble a plan nobody is actually responsible for.

That is exactly why this practice looks the way it looks. I place the implants. We make the teeth in our own lab, down the hall. There is no hand-off because there is nobody to hand it off to. If you have been collecting opinions for six months, save this for the next time someone tells you to get one more.

The biggest myth we deal with around sedation is that it's this one big dramatic thing where you're completely out and someone wheels you in on a stretcher, and that just isn't what it is for most people.

Sedation in our office is really a spectrum, and the right option depends on the patient, the procedure, and how their body actually responds to things, which is something we figure out together before anything is scheduled.

A lot of patients come in convinced sedation doesn't work on them, usually because they had nitrous once for a filling and felt like it didn't do much, and they assume that's the ceiling.

It isn't.

Nitrous, oral sedation, and IV sedation are all different tools for different situations, and matching the right one to the right patient is most of the work.If you've been avoiding treatment because you think the only options are white-knuckle it or get knocked out completely, neither of those is the actual choice.

There's almost always something in between that fits, and we'd rather have that conversation with you than have you keep waiting.

Almost every patient who's been away for a while walks in already apologizing, and honestly, it's one of the hardest parts of the job to hear.

They tell us how long it's been before we've even asked, they list everything they should've done differently, and a lot of them are bracing for some kind of lecture about flossing or waiting too long or letting things get to this point.

That's not what we do here, and it never has been. If you've been putting it off for a year or for ten years, it really doesn't matter to us, because the only thing that actually matters is that you decided to come in.

You don't have to have a plan figured out, you don't have to know what you want done, and you don't need to explain why it's been a while. You can just book the appointment and we'll take it from there together.
