Distal bite is an occlusal disorder in which the upper jaw is significantly positioned forward relative to the lower jaw. Patients with this condition face not only aesthetic issues but also functional impairments, such as difficulties in chewing, breathing, and articulation.

The treatment of distal bite in complex cases includes orthognathic surgery. This is a comprehensive intervention aimed at correcting the relationship between the jaws. During the procedure, the position of either the upper or lower jaw is altered to achieve proper occlusion.

Key points of treatment:
✅ Orthodontic preparation: prior to surgery, it is often necessary to align the teeth using braces or aligners.
✅ Orthognathic surgery: during the operation, the surgeon moves either just the lower jaw (advancing it forward) or both jaws to create a harmonious bite. Special titanium plates and screws are used to secure the jaws in their new position.

Colleagues, let’s discuss the significance of the detailed anatomy of fissures in our work. As is well known, fissures can be classified into first, second, and third order, and their accurate reproduction plays a crucial role in the success of restorations for posterior teeth.

Advantages of detailing anatomy:
- Natural aesthetics. Properly delineated fissures allow for the creation of a restoration that closely resembles the natural structure of the tooth.
- Functionality. Accurate anatomy contributes to the proper distribution of masticatory forces, reducing the risk of failure for both the restoration and the natural tooth tissues.
- Prevention of pathologies. Correct restoration of anatomy decreases the likelihood of developing caries in the fissure areas.

Disadvantages:
- Labor intensity. Recreating complex anatomy requires more time and skill.
- Increased cost. The use of more detailed techniques may raise the treatment cost for the patient.
- Risk of hypercorrection. Fissures that are too deep or sharply defined can complicate oral hygiene and lead to plaque accumulation.
