Same day extraction of infected tooth with cantilevered ceramic bridge delivery with crowding correction. In-office blood draw for L-PRF (clot) fabrication secured with sutures stopped bleeding within minutes on an anticogaulated patient with an INR of 3.6, veneers to follow to brighten remaining front teeth.

“My tooth is sore after eating soy nuts”.…dx fractured tooth ☹️. Blood draw for fabrication of L-PRF plugs/clots helped stop bleeding after surgical removal of the root on this patient on anticoagulants. 3-unit ceramic bridge made using cad/cam tech, all in a single visit.

Teeth are normally suspended in the socket by the periodontal ligament (PDL). Ankylosed teeth however, lose the PDL and become fused to the jaw bone making extraction a challenge. Upper second molar surgically extracted after sectioning and careful bone removal.

COVID cracked tooth syndrome. Yes, the stress is real. Since opening the office, we have been diagnosing/treating 1-3 cracked teeth per week, most are salvageable but some are hopeless and need to be extracted.

“In order for dentistry to do its part to mitigate the spread of COVID-19, the ADA recommends dentists nationwide postpone elective procedures for the next three weeks. Concentrating on emergency dental care will allow us to care for our emergency patients and alleviate the burden that dental emergencies would place on hospital emergency departments.”

That being said, this split/fractured tooth was evaluated yesterday when a patient reported severe pain on chewing. After removal of the tooth ‘splinter’ it was determined that no immediate treatment was needed. Final treatment will be pursued in the future when the unprecedented and extraordinary circumstances of this COVID-19 crisis improves.

Long in the tooth i.e., getting ‘old’ is a term that describes recession of the gums. Exposed root surface is then susceptible to damage from plaque/bacteria, mechanical damage and erosion from acidic environments exacerbated by a dry mouth. While composite restorations may look good, in certain situations, dental amalgam silver fillings are predictable and conservative when done right!

Cracked bicuspid tooth, with obvious crack adjacent to an old amalgam. Tooth also has a perfect gold foil restoration done 40+ yrs ago. Removing the gold inlay and preparing a crown to the gun line can be done but patient was advised resin cement bonding to root margins is not as strong as enamel bonding. The patient chose the conservative approach for functional predictability and was not concerned about esthetics in an area not visible when smiling.

Fractured cusp of a bicuspid tooth down to the gum line with only 1/3 of the tooth remaining presents challenges for crown adhesion and predictability. Conservative tooth preparation that preserves any valuable remaining enamel allows a ceramic ‘crownlay’ to bond to the tooth with zero aesthetic compromise. .MaxCeramic

Surgical extraction of an unrestorable lower 2nd molar with a vertical fracture. What the radiograph did not show was a mini-3rd root! Bone graft placed for a future implant.
