There's a lot of dental work that isn't actually permanent.

I tell every patient this, because I think it changes the way you make decisions.

Crowns fail. Fillings fail. Implants need maintenance. Even the best work I do has a lifespan, and that lifespan depends partly on the materials, partly on my technique, and partly on what you do at home for the next ten or twenty years.

Nothing I put in your mouth will be as strong as what your body originally gave you.

That's why I practice conservatively. The less I have to remove, the more of your natural tooth is still doing the work. The puzzle pieces I add should support the tooth, not replace it.

It's also why I talk so much about home care, nutrition, and what's happening in the rest of your body. Because if I do excellent work and you don't take care of it, the work will fail. That's just biology.

A patient came in not long ago and asked me to remove a root canal tooth.

Her functional medicine doctor had connected some of her chronic symptoms to the dead tissue from old root canal work. She'd done a lot of research before she ever called my office.

Her question wasn't whether to remove it. Her question was whether I'd do it the right way.

A root canal extraction done sloppily can leave ligament fibers behind in the socket. The body sometimes responds by forming a hollow inflamed space in the bone, what's called a cavitation. For patients already managing inflammation in their body, that's the last thing they need.

I do these extractions slowly. I remove the tooth and the ligaments. I clean the site thoroughly. I place PRF, made from your own blood, to support healing from the inside out.

I'm a dental assistant here at Essential Dental, and I'm also studying to become a dentist myself.

Most people are scared of the injection, not the dentistry.

I'd guess at least half of dental fear has nothing to do with the actual procedure. It's the moment right before, when you see the syringe coming and your whole body tenses up. I get it.

In my office, we don’t brush off your fears. They are real.

What we do instead is acknowledge them and work through them with gentle techniques that are soothing and serve as a little distraction so that fear can subside... and you can get the care you really need.

I don't use nitrous. I'll prescribe Va**um if a patient really needs it. But honestly, most of the time, our gentle, effective techniques are enough.

I had a patient who was dead set against having epinephrine in her anesthetic. And she needed a significant repair.

That's the standard mix, local anesthetic plus epinephrine, which keeps the numbing in place longer. But she didn't want it. She'd read that epinephrine can delay healing, and for her own reasons, she wanted it left out.

I told her honestly: without epinephrine, the anesthetic is going to wear off faster. We may have to stop and re-inject during the procedure. It's going to be harder for both of us.

She decided to go through with it anyway.

I respected that decision, the same way I respect every patient who comes in informed about what they want. My job isn't to override your choices. It's to give you accurate information about the trade-offs and then work with you to make it happen safely.

She held herself together through the whole thing. So did I.

Veneers aren't always the answer.

A patient comes in unhappy with their smile, and at most cosmetic-focused dental offices, the conversation goes straight to veneers. Eight of them. Ten. Sometimes more. The teeth get filed down significantly, and there's no going back.

I have a different conversation first.

What's actually bothering you? Are there small black triangles between your teeth from gum recession? We can close those with a system called BioClear and zero tooth removal. Are your teeth a little too short, a little too uneven? We can build them up with bonding that looks natural and preserves your tooth structure. Want them whiter? Whitening alone might be enough.

Veneers are a real option when they're the right one. But they shouldn't be the first option just because they cost the most.

You should always know what's reversible and what isn't before you sit in the chair.
